ObjectiveTo study the relationship and correlative factors of urinary albumin/creatinine(UACR)and oculopathy on patients with type 2 diabetes in order to provide early diagnosis and scientific therapy for clinical management of diabetes.MethodsThe clinical data of 472 patients from June 2015 to June 2016 in the Department of Endocrinology of Anhui Provincial Hospital who were diagnosed as T2DM were analyzed retrospectively.According to UACR more than 30mg/g standards,the patients were assigned into two groups:increased UACR group and normal UACR group.Based on oculopathy,the patients of every group were assigned into three groups:diabetic retinopathy(DR)group,cataract group and normal group,and finally statistically analyzed the morbidity of all complications.Observing general patients data and laboratory examinations including gender,age,duration of diabetes,blood pressure,body mass index(BMI),glycosylated hemoglobin(HbAlc),triglyceride(TG),total cholesterol(TC),uric acid(UA),blood urea nitrogen(BUN)and urinary albumin/creatinine(UAlb/Cr)of both groups,they were analyzed whether there were differences among different groups.The statistical analysis was performed by SPSS for windows 22.0.Results(1)The morbidities of increased UACR,DR and cataract were 35.38%,5.30%and 35.59%in all 472 patients.(2)There was no patient who suffered from DR in normal UACR group,and the morbidity of DR was 14.07%in increased UACR group.The morbidities of cataract were 30.49%in normal UACR group and 44.91%in increased UACR group,and there were statistical differences(P=0.028<0.05).(3)? duration of diabetes:There were statistical differences between normal UACR group with increased UACR group(P=0.025),and the latter of which was obviously prolonged.In patients who suffered from increased UACR,compared with the normal group,the durations of diabetes were significantly prolonged in the cataract group and diabetic retinopathy group,and the differences were statistically significant(P=0.001 and 0.002).There was no difference between normal group in patients who suffered from increased UACR with normal UACR group(P=0.699),suggesting that elevated UACR in some patients may not have a close relationship with DM.? age:There was no difference between increased UACR group with normal UACR group(P=0.054),and there were statistical differences among DR,cataract and normal groups in patients who suffered from increased UACR.The age of cataract group was significantly larger than that of DR group,and that of DR group was larger than that of normal group.? blood pressure:There were statistically significant differences between increased UACR group with normal UACR group(P=0.002),and there was no difference among DR,cataract and normal groups in patients who suffered from increased UACR(P=0.477).?BUN:There was no difference between increased UACR group with normal UACR group(P=0.08).There were statistically significant differences in patients who suffered from increased UACR between DR group with normal group(P=0.006),and DR group with cataract group(P=0.007),but there was no difference between normal group with cataract group,what suggested that there was a certain correlation between renal function with retinopathy,but no correlation between renal function with cataract in diabetic patients.? gender,body mass index(BMI),glycosylated hemoglobin(HbAlc),triglyceride(TG),total cholesterol(TC),uric acid(UA):There was no difference among all groups.?UACR:There was no difference among DR,cataract and normal groups in patients who suffered from increased UACR.Conclusion(1)The morbidity of increased UACR in type 2 diabetic patients is much higher than that of DR.Both of them increasing with the extension of the course of disease,but they are not correlated with gender and BMI.(2)There are certain increases of urinary albumin excretion in patients with diabetic retinopathy,suggesting that the increase of urinary albumin/creatinine may predict the occurrence of diabetic retinopathy.(3)The morbidity of cataract in diabetic patients is related to the extension of the course of disease and the age of the patients,but not correlated with renal function.(4)Hypertension is an important risk factor for kidney damage,but not related to diabetic eye disease.
