Objective:With the development of economy and the improvement of people's living standard, the mortality rate of chornic kidney disease increase obviously and become a major public health threat worldwide.lt is reported that the prevalence rate of chronic kidney disease is approximately 10.8%in our country,in other words,As many as one in ten Chinese suffered chornic kidney disease. The Mineral and Bone Disorder in Chronic Kidney Disease is a common complication of CKD cases that severely affect the life quality and survival rate of patients, and it is also one of common death causes of CKD patients. The Fibroblast Growth Factor 23(FGF23)is a recently discovered hormone, recent research has shown it participate in the process of bone disorder and as a predictor in the progression of CKD.However,It is not completely elucidated about the relationship beween bone metabolism and FGF23.The topic is to elucidated the interaction between FGF23 and CKD,then, provide a new ideal to the prevention of Mineral and Bone Disorder in Chronic Kidney Disease. Methods:We recorded 115 patients with chornic kidney disease that not dialysis.Exculding Primary parathyroid diseases,Primary bone metabolic diseases, fracture and tumor.According to the GFR,patients can be divided into CKD 1-2 stages(32cases),CKD3-4 stages(40cases),CKD5 stages(43cases).The levels of FGF23,25-OH-VD,BAP and TRACP-5b were measured by ELISA.Data including basic clinical information(sex, age,Drug programs)and blood biochemical values(Scr,BUN,CYS-C,Ca,P,ALB,PTH,ALP)were collected. Then,Compareing the content of these Indicator. And The Pearson correlation analysis were used to identify the relationship between FGF23 and calcium, phosphate and biochemical makers of bone turnover. Results:The levels of FGF23,BAP,TRACP-5b increased gradually with the decline of renal function, whereas, the 25-OH-VD reduced. There were significant difference between CKD5 stage and CKD1-4 stage(all P<0.05).The correlations showed that FGF23 was positively correlated with BAP,TRACP-5b,serum phosphoru,Scr,BUN,CYS-C,and was negatively correlated with 25-OH-VD,serum calcium(P<0.05).Multiple linear regression analyse demonstrated that Scr and serum phosphate were the independent fators affecting FGF23 in the total CKD patients.Conclusion:FGF23 is closely related to the progression of renal function and bone metabolism disorder in patients with chronic kidney disease, which may be a risk factor for the progression of bone metabolism disorders and renal dysfunction in CKD patient.
