Objective:This thesis is concerning about assessing the guidance for the classification for hip fracture of aged patients and prosthesis selection of artificial femoral head replacement.For aged patients with common femoral neck and femoral intertrochanteric fractures,we put them collectively known as a hip fracture.we define the classification for hip fracture of aged patients based on the damage for calcar femorale,the objective of this thesis is to provide varies prosthesis treatment and follow-up for varies hip fracture and provide reference for hip fracture of aged patients when we perform artificial femoral head replacement foe these patients according to the study of our group.Method:from January 2012 to October 2016,our group have collected and analyzed 55 cases of artificial femoral head replacement for hip fracture of aged patients more than eighty years old.During these cases,patients are aged 80-97 years old.22 cases are males,33 cases are females,the average age of these patients are 87 years old.According to the patients' imaging data before operation,which shows the fractures damage for calcar femorale,all the patients were divided into several types:type A,typeB1,typeB2,type C,and choose corresponding femoral prosthesis.We give all patients performed artificial femoral head replacement therapy,and apply X-ray observations to watch the fracture healing and combine with Harris to score,to follow-up regularly.Assess all postoperative patient recovery and clinical curative effect,finally respectively statistical comparison of type A type B(B1 and B2)in patients with type C fracture group Harris score difference before and after operation are the same.Result:The present study was carried out after the regular follow-up of 55 patients,up from 4 to 52 months(mean 18 months)?X-ray sheets show the location and angle of the prosthesis were well,and postoperative lower limb had a nearly equal length compared with the contralateral lower limb while there were 2 cases shortening above lcm.Harris scores 65 to 91 points,an average of 79.3 points,excellent rate of 100%.The difference between the 3 types were statistically significant(P<0.05),while the order of scores different was C type>B type>A type.Conclusion:For this kind of hip fracture in aged patients,surgeon need not only technical operation for the successful of the Artificial femoral head replacement technical operation,selecting suitable prosthesis is the key.As a result,we develop a new comprehensive classification for hip fracture,which only describe a core concern of hip fracture—integrity of femoral spur,to provide more intuitive advice for surgeon to select prosthesis.From this study for postoperative recovery of patients can we see,this king of classification is fairly feasible.
