Curative Effect Observation On Abdominal Acupuncture And CPM Machine Training After Total Knee Arthroplasty

Abstract/Summary:
ObjectiveThe research objectively examines the curative effect and safety of abdominal acupuncture and CPM machine training in early rapid rehabilitation management after TKA through an observation of the respective clinical effect of abdominal acupuncture,supplemented with early rapid rehabilitation training,and CPM training,supplemented with early rapid rehabilitation training,on TKA patients.Meanwhile,the author observes the respective significant curative effect of abdominal acupuncture and CPM on TKA patients.By means of optimizing the combination of early rehabilitation measures,the author expects to find one rehabilitation program combining Chinese and Western medicine,which has affirmative curative effect and is easy to practice,to conduce toward further clinical application.Last but not least,the author also discusses the mechanism of his proposed program in this thesis.MethodsIn this study,prospective randomized controlled trials were used.Using a random number table method.Select the orthopedic patients of Guangdong Provincial Hospital of Traditional Chinese Medicine who meet the criteria of this study,96 patients from August 2013 to December 2015 line of TKA surgery were randomly divided into CPM group(n=32)、abdominal acupuncture group(n=32)and control group(n=32),of which 7 cases of control group off,other patients were successfully completed the experiment,89 cases of effective cases were collected.Patients of the control group received routine early rehabilitation training while patients of the CPM group、abdominal acupuncture group,in addition to the routine early rehabilitation training,separately received CPM machine training、abdominal acupuncture.CPM group、abdominal acupuncture group received 12 treatment.Results of scores in three respects including 3days、7days、12days training of HSS、VAS、range of motion(ROM)were compared between the three groups.The rating scale was adapted from the Knee Scoring System of the U.S.Hospital for Special Surgery(HSS).Statistical methods:baseline analysis was performed on the data of this study.The chi-square test was used to compare the count data.When the comparison between the measurement data group,in line with the normal distribution and homogeneity of variance characteristics,the use of variance analysis.In the case of homogeneity of variance or non-normal distribution,the nonparametric test is used for the analysis.ResultsOf the 89 subjects,11 are male while 78 are female,33 received TKA on the left side while 56 on the right side and 45 are diagnosed as primary one-knee arthritis while 44 primary two-knee arthritis.The subjects’ age range is from 47 to 85.The differences in the baseline conditions of subjects,including age,gender,preoperative diagnosis,surgical site,preoperative HSS score,preoperative ROM score and preoperative VAS score,are not statistically significant(P>0.05)and therefore the subjects are comparable.The VAS score,ROM score and HSS score of the CPM group are higher than those of the control group(P<0.05)after a 3-,7-and 12-day treatment period.The VAS score of the abdominal acupuncture group is higher than that of the control group after a 7-and 12-day treatment period.However,the VAS score of the abdominal acupuncture group,compared with the control group,is not statistically significant after a 3-day treatment period,which probably results from the limited number of subjects or deficiency in the accumulative curative effect of acupuncture therapy.The result suggests an increase of the number of subjects in future researches.No adverse event occurs during the process of the author’ s experiment.ConclusionEarly rehabilitation therapy turns out to have significant curative effect on TKA patients in relieving postoperative pain and improving ROM and HSS functions.However,the curative effect of CPM training is better than that of early rehabilitation therapy and abdominal acupuncture in the respects above.And,after having been treated for 7 days and 12 days with abdominal acupuncture,its curative effect outperforms that of early rehabilitation therapy in pain relief.In brief,the application of abdominal acupuncture and CPM machine training both effectively help the recovery of the function of the patients’ knee joint after TKA.All the 3 therapeutic programs are counted as reliable and safe.
