Objective:The parameters quantitatively of strain imaging and echocardiography about tuberculous constrictive pericarditis was summarized to explore its ultrasound diagnostic value.Methods:Object:Patients were divide into 2 groups:the 17 cases with tuberculous constrictive pericarditis(CP)and the 20 cases of normal control group,ultrasonic parameters of patients in two groups were analyzed.We Measured the thickness of the pericardium,biplanar Simpson method measuring left ventricular LVEF,left ventricular end-diastolic volume;Apex four cavity section measuring right ventricular end-diastolic diameter(RVIDd),Simpson method to measure the right ventricular end-diastolic area and end systolic area,calculate right ventricular area change rate;Pulse doppler measurement of mitral valve and tricuspid valve mouth E peak,A fengfeng value speed,calculation,E/A ratio measurement E peak with the breathing rate;Tissue doppler imaging measurement of mitral valve,tricuspid valve E,A'and' S',and calculate the E'/A',the left ventricle and right ventricle Tei index;M type echocardiography measured mitral valve and tricuspid annulus systolic displacement(MAPSE,TAPSE).We measured inner diameter of the inferior vena cava at xiphoid process under section and calculated the breathing rate.Data collection:Two-dimensional gray-scale consecutive 3?5 cardiac cycle,frame frequency 50?70 frames per second were collected and storaged with the standard apex four-chamber view dynamic image.Images can clearly show endocardial surface outline of the left ventricle,right ventricular for offline analysis software.Data analysis:The dynamic image was imported the QLAB 9.1 analysis software and selected CMQ mode.We choosed a cardiac cycle,respectively sketched left ventricular endocardial myocardium(inner heart muscle)and the epicardial myocardium(outer myocardial).The software automatically generated interest area,and the left ventricle was divided into six segments,according to the tracking,operation calculation program can get left indoor layer myocardial,long axis of the outer myocardial strain,circumferential strain and torsion Angle.Statistical methods:We used SPSS 16.0 statistical software,measurement data mean comparison between groups was used independent sample t test method,the differences were statistically significant for P<0.05.Results:The changes of CP group have obvious changes compared with control group,such as MAPSE,EMV,,EMV respiratory rate,LV Tei index,especially the EMV respiratory rate and LV Tei index.The changes of CP group have obvious changes compared with control group,such as TAPSE,RV area rate(%),ETV,RV Tei index,ETV respiratory rate(%),IVC respiration rate(%).Compared with control group,overall torsion,endometrium torsion,reverse the outer membrane and reverse cross the wall of CP group ere reduced,The reverse of outer membrane was obviously reduced than the lining reverse in CP group.Parts of outer cardiac and pericardia in some patients have adhesion,no significant movement.The inner myocardial,outer myocardial and whole long axis strain of CP group reduced.In CP group,compared with inner heart,myocardial strain rate of long axis in outer myocardium reduced.The inner myocardial,outer myocardial,whole circumferential strain reduction of CP group reduced.The circumferential strain of outer myocardium in CP group decreased to inner heart.Conclusion:Echocardiography and Qlab analysis software can effectively improve the diagnosis of tuberculous constrictive pericarditis,Strain imaging and echocardiography parameters in quantitative research of patients with tuberculous constrictive pericarditis has important value.
