ObjectivesThrough the analysis of the distribution regularity of TCM constitution in the patients with coronary heart disease and chornic heart failure and the correlation between constitution and the cardiac function indexes.This paper discusses the characteristics of TCM constitution,to provide evidents of the inervention of the diease for improving the level of syndrome differentiation and treatment.MethodsA cross-sectional study was performed,120 cases of patients with coronary heart disease and chronic heart failure were selected.Collected the data of the patients,including general information,New York Heart Function Assessment,NT-proBNP,results of cardiac color Doppler ultrasound and TCM Constitutional Types.Then analyzed the correlation between the data colleted above and the TCM constitutions.Results1.Of all the 120 cases of coronary heart disease with chronic heart failure,biased constitution accounted for 95.8%.The top four constitutions were qi-deficiency constitution(25%),static-blood constitution constitution(20%),yang-qi deficiency constitution were(18.3%)and phlegm-dampness constitution(17.5%).2.There were statistical differences in the gender and age of patients with coronary heart disease complicated chronic heart failure.Coronary heart disease complicated chronic heart failure in men gave priority to qi-deficiency,phlegm wet quality,while the female patients with static-blood constitution and yang-qi deficiency constitution(P<0.05),the constitutions of the patients between 61-70 years old are mostly qi-deficiency constitution,while 80 years old patients are mostly yang-qi deficiency constitution(P<0.05).3.In patients with coronary heart disease complicated chronic heart failure of the common constitution types,qi deficiency gave priority to NYHA class ? and ?,static-blood constitution and phlegm-dampness constitution gave priority NYHA class III,yang-qi deficiency constitution gave priority to NYHA class ?(P<0.05).4.There were significant differences in EF and FS levels between different groups of patients with coronary heart disease complicated chronic heart failure(P<0.05,P<0.05).5.There was no significant difference in LVEDD,LVESD and LA diameter among different common constitution types of patients with coronary heart disease complicated chronic heart failure(P<0.05,P<0.05,P<0.05).6.In patients with coronary heart disease complicated chronic heart failure of the common type of constitution,the yang-qi deficiency constitution had the highest level of NT-proBNP,followed by static-blood constitution and phlegm-dampness constitution,the level of NT-proBINP in patients with qi-deficiency constitution was lowest(P<0.05).Conclusions1.TCM Constitution typing of the patient with coronary heart disease complicated chronic heart failure were characterized by qi-deficiency constitution,static-blood constitution,yang-qi deficiency constitution and phlegm-dampness constitution,suggesting that the four types of physical constitution were the most important types.2.There were statistical differences in the gender and age of patients with coronary heart disease complicated chronic heart failure.3.The distribution of heart function classification was different in patients with coronary heart disease complicated chronic heart failure of the common constitution types.The patients of Yang-qi deficiency constitution tended to increase with the increase of heart function classification.4.The level of EF,FS were different in patients with coronary heart disease patients complicated chronic heart failure of the common constitution types.The EF,FS levels were the highest in qi-deficiency constitution,phlegm-dampness constitution and static-blood constitution were decreasing gradually,the levels of EF and FS in yang-qi deficiency constitution were the lowest.5.The level of NT-proBNP in patients with coronary heart disease complicated chronic heart failure was different.The level of NT-proBNP increased gradually as the type of constitution changed from qi-deficiency constitution,phlegm-dampness constitution,static-blood constitution to yang-qi deficiency constitution. |
