The Survival And Prognosis Of Three-dimensional Radiotherapy With Concurrent Chemotherapy For Squamous Carcinoma Of Stage ? Non-small-cell Lung Cancer

Objective:To analyze the survival and prognostic factors Squamous Carcinoma of stage ? non-small-cell lung cancer treated three-dimensional radiotherapy with concurrent chemotherapy.Methods:The patients from two prospective phase ? study.101 cases of 88 were included pre-protocol.Patients received platinum-doublet chemotherapy with concurrent irradiation to the primary tumor.Primary endpoints were overall survival(OS)and progress-free survival(PFS).Survival was calculated by Kaplan-Meier method and compared using the Log-rank.Cox regression model were used to prognostic analysis on overall survival.Results:Of 88 patients,the 1-,2-,3-and 5-yrs OS and MST were 42.2%?13.6%?8.7%?3.1%and 10ms,respectively.With the dose of primary tumors increases,OS and PFS were improvement gradually.The 1-,2-,3-and 5-yrs OS and median survival time(MST)of PTV dose?63Gy vs.<63Gy were 45.7%vs.39.6%,25.7%vs.4.5%,17.1%vs.2.8%,7.1%vs.0%and 11ms vs.10ms(?2= 7.286,P=0.007)respectively,the median PFS 9ms and 7ms(?2=4.574,P=0.032),there was significant difference between two groups.The 1-,2-,3-and 5-yrs OS and PFS for the patients who received 4 cycles concurrent chemotherapy and dose of primary tumors>63Gy were 51.9%,29.6%,18.5%,9.9%and 9months(?2=10.204,P=0.001 and??2=6.343,P=0.012)?Muhivariate analysis showed that PTV,the number of metastatic lesions,the change of intensity,KPS before and after treatment is to improve the independent prognostic factors for OS.Conclusion:Concurrent chemotherapy and three-dimensional radiotherapy with squamous carcinoma of stage IV non-small-cell lung cancer could prolong survival,as the strength with the comprehensive treatment improve the OS and PFS gradually prolonged.The number of metastatic lesions,the change of intensity,KPS before and after treatment is to improve the independent prognostic factors for OS.
