ObjectivesTo analyze the related influencing factors which cause systemic inflammatory response syndrome(SIRS)after percutaneous nephrolithotomy(PCNL)in order to get better clue to the prevention.MethodsMedical records of 190 patients who were treated with PCNL from January 2014 to October 2016 in Qilu Hospital of Shandong University were reviewed retrospectively according to gender,age,diabetes history,hydronephrosis,duration of surgery,size of stones,single or multiply stones and preoperative midstream urine culture.Descriptive statistical analysis,single-variable analysis and multiple Logistic regression analysis were used in the analysis of statistics.ResultsA total of 190 patients were enrolled in this study and 40 patients developed SIRS(21.1%).Single factor analysis indicats that diabetes history(p=0.023),duration of surgery(p=0.034),size of stones(p<0.001)and preoperative midstream urine culture(p<0.001)are important factors indicating postoperative SIRS(p<0.05);while there were no statistically significant differences among gender,age,hydronephrosis,and single or multiple stones.Multivariate Logistic regression model indicated that diabetes history and size of calculi are independent risk factors of SIRS after PCNL.ROC curve shows that the two factors have certain Joint predictive value(AUC=0.743,95%CI:0.653-0.833).ConclusionsDiabetes history and size of stones are independent risk factors of SIRS after PCNL,and urologist should pay attention to them.
