| ObjectiveApplications PiCCO monitoring hemodynamic parameters of patients with septic shock,to assess the effects on hemodynamics and prognosis in patients with septic shock using the Shenfu injection.MethodJanuary 2012 to January 2014,40 cases of septic shock patients admitted to our hospital ICU were randomly divided into control group and treatment group,20 cases in each group,two groups of patients according to 2012 guidelines cluster of treatment,treatment group plus the Shenfu injection 100ml,the control group were treated with saline 100ml,continuous medication for seven days.All patients were monitored hemodynamic parameters application PiCCO and changed blood lactate levels,comparing two groups of patients before and after treatment 6h,24h,3d,7d of blood lactate and hemodynamic parameters,and observed the 7d and 28d survival and ICU length of stay.Results6h after,treatment patients with LAC level dropped to 4.1±2.0mmol/L,the control group was 5.1±2.3mmol/L,compared with before treatment decreased significantly(P=0.012 and 0.009),7d after,LAC level of treatment group of patients was significantly lower than that of the control group(2.1±1.2mmol/L and 3.2±2.5mmol/L,P = 0.027).6h after treatment,the GEDI,CI and SVRI of two groups of patients significantly increased than those of before treatment(P<0.05),PVPI and EVLWI were no significant difference(P>0.05).3d and 7d after treatment PVPI and EVI.WI of two groups were significantly decreased than those of before treatment(P<0.05),and 7d after treatment,the CI,PVPI and EVLWI of treatment group of paatients were significantly better than those of the control group(P<0.05).The treatment group of patients with ICU length of stay was significantly less than the control group(15±11 and 19±14,P =0.027),there was no significant difference in mortality 7d of two groups of patients(25%and 30%,P = 0.375),but 28d mortality,the treatment group was significantly lower than the control group(30%and 45%,P =0.044).ConclusionThe Shenfu injection improved microcirculation and the patient’s heart function,reduced vascular permeability and the extravascular lung water to reduce mortality in patients with septic shock. |
