| BackgroundThe outcome of early researches has showed that REDD1’s expression is higher in a variety of human organs gland cancer cells lines of Ras-transformed and it is a key in the process of Ras malignant transformation.In many kinds cancer cells of human the PI3K/AKT pathway can be activated and lead to growth、proliferation and survival of them.But related reports about cardia adenocarcinoma have not yet been seen.ObjectiveResearch the relation of the expression of REDD1,KRAS gene mutation,the PI3K/AKT pathway and clinical pathological and prognosis of cardia glandular cancer case.MethodsCollect our department 78 cases with cardiac adenocarcinoma tissue,17 cases of high grade glandular intraepithelial neoplasia,16 cases of normal cardiac mucons tissue samples,testing the RAS gene mutation using PCR and direct sequencing probe method;Detection of REDD1 and p-AKT expression by immunohistochemical method;Statistical analysis,By SPSS 17.0 statistical software for statistical analysis,the difference between each rate using the variance test,P < 0.05,with statistical significance.Results(1)we have detected KRAS mutations of 78 cases with cardiac adenocarcinoma tissues,16 cases are positive,mutation rate20.5 %;17 cases of high grade glandular intraepithelial neoplasia organization,5 cases are positive,Mutation rate 29.4%;16 casesof normal cardiac mucous tissue,1 case positive for mutation rate 6.25%;The extent of differentiation of cardia adenocarcinoma is associated with KRAS mutations,the mutation rate of the low grade carcinoma is significantly higher than the high grade(p<0.05).(2)The rate of high expression of P-AKT protein in cardiac adenocarcinoma is 43.6%(34/78),Higher than normal cardiac mucosa tissue(18.7%)and high grade glandular intraepithelial neoplasia(17.6%)(P<0.05).(3)The low differentiationa and late clinical stage of cardia adenocarcinom and p-AKT high expression is positive correlation;p-AKT protein expression in the group of resistance to chemotherapy drug is higher than the effective group(P<0.05);the group of P-AKT high expression is lower than that of lower expression in terms of overalsurvival and disease-free survival(P < 0.05).(4)REDD1 high expression rate in cardiac adenocarcinoma(47.4%)is obviously higher than normal cardiac mucosa tissue(12.5%)and high grade glandular intraepithelial neoplasia(17.6%)(P<0.05).(5)There is a positive correlation between p-AKT and REDD1 protein expression in cardiac adenocarcinoma(P< 0.05).Conclusion(1)In the process of cardia adenocarcinoma occurs,is often associated with KRAS gene mutations,and the mutation rate is higher in the low grade glandular cancer.(2)There is obvious positive correlation between REDD1 high expression and p-AKT expression,and there is a positive relation among high grade 、late clinical stage、 resistance to radiation and chemotherapy、poor prognosis and the two factors. |
