| Objective To explore and compare the efficacy of conservative and surgical treatment of grade-Ⅱ medical collateral ligament injury after the reconstruction of the repture of anterior cruciate ligament of knee.Methods 43 cases selected from September 2013 to October 2014 in our hospital,all patients with unilateral anterior cruciate ligament rupture and grade-Ⅱ medical collateral ligament injury were strict randomly divided into two groups marked A(odd number) and B(even) according to the surgical sequence.The group A had 22cases(including 13 males and 9 females),the average age was 33.5(ranged from 21 to 39);The group B had 21 cases(including 14 males and 7 females),the average age was 32.8(ranged from 19 to 38).All patients were measured by KT 1000 instrument for anterior tibial translocation and assessed by Lyshlom score scale before surgery.The treatment of group A was anterior cruciate ligament reconstructed by achilles tendon allograft and medial collateral ligament treated by conservative method;The approach of group B was anterior cruciate ligament reconstructed by achilles tendon allograft and medial collateral ligament treated by operative way.To record the hospital day and wound healing of A and B group.One year after surgery,to record the value of tibia being relative to the femur forward,Lyshlom scores and valgus stress test examination results.Results Two patients were lost in group A, the remaining patients were followed-up, one patient was lost in group B,the others were followed-up,the follow-up time of two groups was one year. There was no cases of wound infection dury the postoperative hospital stay in group A,three cases of wound infection in group B.Compared with the hospital stay,there was statistical significance between two groups(p>0.05). Compared with the KT-1000 measurements and Lyshlom scores of group A in the preoperation and postoperation,there was apparent statistical significance(p<0.05);Compared with the KT-1000 measurements and Lyshlom scores of group B in the preoperation and postoperation,there was apparent statistical significance(p<0.05). Compared with the KT-1000 measurements and Lyshlom scores between group A and B one year after surjury,there was no statistical significance(p>0.05).One year after surjury,the valgus stress test examination results were nagtive of two groups.Conclusion There were no significant differences in clinical efficacy about knee AP stability and functional improvement when the grade-Ⅱ medial collateral ligament injuries wered treated by conservative or surgical approach after the reconstruction of the repture of anterior cruciate ligament.Both two treatment methods can achieve a satisfactory clinical efficacy.However,compared with surgical treatment, conservative treatment had a short hospital stay and a low incidence of wound infection,therefore,it is recommened that the grade-Ⅱ medial collateral ligament injury should be treated by conservative approach after the reconstruction of the repture of anterior cruciate ligament of knee. |
