Objective:Meniere's disease is a dynamic progressive disease,with the different development of the disease, there would be different pathological changes in different periods of the disease which may lead to different clinical features. The clinical manifestations was so perplex that it was difficult to diagnosis and distinguish diagnosis of Meniere's disease rely on clinical symptoms only. electrocochleography as one of the important methods in diagnosis of Meniere's disease, which provided an objective basis for clinical diagnosis for Meniere's disease. However, electrocochleography in different stages of the pathogenesis of Meniere's disease and its diagnostic value has obvious difference. The purpose of this study was to explore in Meniere's disease attack and intermittent period for electrocochleography and significance of electrocochleography SP/AP area ratio with respect to sensitivity the diagnosis of Meniere's disease than SP/AP amplitude, and the calculation of the normal control group electrocochleography SP/AP amplitude ratio and area ratio of normal reference value.Method: 42 cases(45 ears) of Meniere's disease patients who were outpatient and inpatient in Tianjin Medical University General Hospital between September 2014 and 2015 June were received electrocochleography examination during acute attacks and quiescence phase. The SP/AP amplitude and area curve ratios were measured respectively(to derive the SP/AP amplitude and area curve ratio). 22 healthy volunteers(44 ears) with normal hearing were selected as the normal control group. and the SP/AP amplitude ratio and the area ratio of SP/AP were examined and the normal critical reference value was obtained. Analysis the results in the normal control group?acute attacks and quiescence phase of Meniere's disease, and analysis of the results of the SP/AP area ratio and amplitude ratio. Analyzed the positive rate of the diagnosis results between SP/AP area ratio and amplitude of the electrocochleography, and analyzed the results of electrocochleography in MD acute attacks and MD quiescence phase and the normal control group,at same time parameter values of SP amplitude, AP amplitude and SP area and AP area during acute attacks and quiescence phase were compared.Result: By t test, the parameter values of SP amplitude, AP amplitude and SP area and AP area during acute attacks and quiescence phase were compared, the parameter values of SP amplitude?AP amplitude ?SP area and AP area during acute attacks are all greater than quiescence phase, the result the differences were statistically significant(P < 0.05). By the F rest, the SP/AP area curve ratio parameters in MD acute attacks and MD quiescence phase and the normal control group were compared, and the difference was significant(F=111.10,P<0.05). There were significant different between the two groups among three(P<0.05).The SP/AP area curve ratio parameters among the MD acute attacks and MD quiescence phase and the normal control group were significant different(F=65.68,P<0.05).There were significant different between the two groups among three(P<0.05). The positive rate which during the period of acute attacks was higher than that quiescence phase period, The positive rate of the SP/AP amplitude and area curve ratios between acute attacks and quiescence phase were significant different(P<0.05). The SP/AP area curve ratio positive rate during acute attacks was 95.56% and SP/AP amplitude ratio was 71.77%.SP/AP area curve ratio positive rate during quiescence phase was 55.56% and amplitude ratio was 33.33%. The results showed that there were significant different between the two markers(P<0.05). Correlation coefficients of SP/AP amplitude ratio and area ratio were 0.65,0.85,0.81.The upper limit of normal for SP/AP amplitude and area curve ratio were respectively 0.40 and 1.78.Conclusion: It can increase the positive rate when undergo the Ecoch G examination during acute attacks of Meniere's disease,it is better to give the examination during the acute attacks of MD, while the SP/AP area curve measures is more sensitive than SP/AP amplitude ratio in diagnosis of Meniere's disease. Amplitude of SP/AP ratio and area ratio is associated. This recording and analysis method for examining can save time and improve the efficiency. It has a very important role. for the early diagnosis of Meniere's disease, early intervention, prevention function of the inner ear further injury.
