Objectives: To analysis the differences in plasma mi RNA-1 and mi RNA-21 between normal control group and stent group and their clinical significance,then on this basis,to study the difference in mi RNA-1 and mi RNA-21 levels in the plasma between coronary heart disease(CHD) patients with in-stent restenosis(ISR) and non-stent restenosis(NISR). To investigate their clinical significance. To study on the mi RNA-1 and mi RNA-21's prediction value in CHD patients with in-stent restenosis. To explore whether the mi RNA-1 or mi RNA-21 is the independent risk factor for the CHD patients with in-stent restenosis.Methods: To collect 195 cases of coronary angiography to rule out the diagnosis of CHD patients as normal control group, Collecting 187 CHD patients with stents who come back for reviewing the coronary angiography(CAG) as stent group. then Collect their plasma, extracting total mi RNAs. The contents of mi RNA-1 and mi RNA-21 were detected using q RT-PCR method, and analyze the differential expression of mi RNA-1 and mi RNA-21 between two groups. among the stent group, there are 48 patients are in-stent restenosis and 139 patients with non-stent restenosis by doing CAG, they were respectively defined as ISR group and NISR group, The contents of mi RNA-1 and mi RNA-21 were detected using q RT-PCR method, and analyze the differential expression of mi RNA-1 and mi RNA-21 between ISR group and NISR group. By using ROC curve(receiver-operator characteristic curve), respectively to explore the prediction value of the plasma mi RNA-1 and mi RNA-21 in CHD patients for ISR. With in-stent restenosis for reference,adopt single factor logistic regression analysis, to explore whether plasma mi RNA-1 and mi RNA-21 respectively as a risk factor for ISR, then we use multiariable logistic regression analysis whether plasma mi RNA-1 and mi RNA-21 respectively as independent risk factors for ISR.Results: 1. The relative expression level of the plasma mi RNA-1 and mi RNA-21 in nomal group are more higher than that of stent group(P<0.05), The difference wasstatistically significant. mi RNA-1 and mi RNA-21 may be independent risk factors for the CHD patients with stent. 2. The relative expression level of the plasma mi RNA-1 and mi RNA-21 in ISR group are more higher than that of NISR group(P<0.05), The difference was statistically significant. 3. By using ROC curve, respectively to explore the prediction value of the plasma mi RNA-1 and mi RNA-21 in CHD patients with ISR: the AUC(Area under the ROC curve)of plasma mi RNA-1 is 0.854,P <0.05. 95%CI(Confidence Interval)is 0.801 to 0.907, When the relative expression level of the plasma mi RNA-1>2.4067 for CHD patients with ISR is the best point of prediction and diagnosis, its specificity is 76.3%,sensitivity is 95.8%. And the AUC of plasma mi RNA-21 is 0.857,P<0.05. 95% CI is 0.767 to 0.948, When the relative expression level of the plasma mi RNA-21>2.9515 for CHD patients with ISR is the best point of prediction and diagnosis, its specificity is 97.1%, sensitivity is 79.2%. 4. With in-stent restenosis for reference,adopt single factor logistic regression analysis, to explore whether plasma mi RNA-1 and mi RNA-21 respectively as a risk factor for ISR:diabetes mellitus,smoking,more smaller stent diameter,more longer stent length,the relative expression level of the plasma mi RNA-1 and mi RNA-21 have statistical significance. then we use multiariable logistic regression analysis whether plasma mi RNA-1 and mi RNA-21 respectively as independent risk factors for ISR:diabetes mellitus,smoking,more smaller stent diameter,more longer stent length, the relative expression level of the plasma mi RNA-21 have statistical significance,but the plasma mi RNA-1 has no statistical significance.Conclusion: 1.The relative expression level of the plasma mi RNA-1 and mi RNA-21 in the patients with stent are more higher than that in nomal people. mi RNA-1 and mi RNA-21 may be independent risk factors for the CHD patients with stent. 2.The relative expression level of the plasma mi RNA-1 and mi RNA-21 in ISR group are more higher than that of NISR group. 3.The plasma mi RNA-1 and mi RNA-21 have a relative high sensitivity and specificity, They may be clinical indicators for predicting CHD patients with ISR.4. High relative expression of plasma mi RNA-21 may be an independent risk factor for ISR, but high relative expression of plasma mi RNA-1may be not an independent risk factor for ISR.
