| Background and aims: Percutaneous microwave coagulation therapy usually has some blindness, It is difficult to measure the puncture for liver cancer in the special situation and easy to cause deputy injury.This objective of our study was to discuss the clinical value of laparoscopic Microwave Ablation in treatment for special situation hepatocelluar carcinoma.Method:A retrospective study was performed on 42 patients undergoing laparoscopic Microwave Ablation for special site liver cancer from September 2006 to September 2015.Comparing with 63 patients undergoing percutaneous microwave coagulation therapy for special site liver cancer guided by ultrasound between July 2009 and January 2015 in tongji hospital. Related indicators of microwave treatment and postoperative complications were compared between the two groups.Result:Two groups had no perioperative deaths, The average microwave treatment time of laparoscopic group(14.5±2.4min) significantly longer than that in percutaneous microwave coagulation therapy(8.3 ±1.6min)(p<0.05),The time of postoperative recovery in patients between laparoscopic group(6.3±2.1d) and percutaneous microwave treatment group(5.8±3.7d) was no obvious difference(p>0.05);The incidence of pleural effusion in the laparoscopic group was significantly lower than that in percutaneous microwave coagulation group(p<0.05),The incidence of gallbladder injury rate of laparoscopic group was significantly lower than percutaneous microwave coagulation group(p<0.05), The rest of the complications in the two groups have no significantly difference.Conclusions :Laparoscopic Microwave Ablation therapy can make up for the defect of the percutaneous microwave coagulation in treatment of liver cancer, especially for special situation hepatocelluar carcinoma. Laparoscopic Microwave Ablation therapy has important clinical application prospect and clinical trearment effect. |
