Objective: This study will explore the relationship between weight gain in early stage of pregnancy and pregnancy outcomes and its influencing factors, which in order to promote the reasonable gestational weight gain, and put forward the measures of management for gestational weight gain. Methods: This study is made up of two parts, and one of them is to examine relationship between weight gain during 5~17 weeks in pregnancy and pregnancy outcomes, which adopted the method of prospective cohort study. In this part, 411 pregnant women from the Central Hospital of Wuhan were followed up from November 2013 to January 2016. Basic demographic characteristics and pregnancy outcomes of the study participants were collected by questionnaires. Then analyzed correlation between the growth rate of body mass index and body weight of pregnant women with 5~17 and the outcomes of delivery by Pearson linear correlation or Spearman rank correlation method. Another part of this study is to explore factors influencing weight gain during 5~17 weeks in pregnancy, which adopted the method of cross-sectional study. From November 2013 to January 2016, 1054 pregnant women from the Central Hospital of Wuhan were urged to complete a questionnaire about basic demographic characteristics and a food frequency questionnaire to estimate dietary intake. Then used multiple linear regression method to analyze the factors influencing body mass index and weight gain rate of pregnant women in 5~17weeks. Epidata software was used to establish database and input data, and SPSS17.0statistical software was used for data analysis. Results: It was a positive correlation between weight and body mass index(BMI)growth rate of the study participants during 5~17 weeks of gestation and gestationalweight gain, birth length and hemorrhage of delivery(P<0.05). Weight and BMI during 5~17 weeks of pregnancy of the pregnant women who delivered macrosomia and fetal umbilical cord around the neck increased rapidly. On the contrary, weight of the ones who delivered low birth weight infant during 5~17 weeks of pregnancy gained slowly(P<0.05). Compared participants whose body mass index during 5~17weeks in pregnancy increased more than 0.01kg/m2 per week, the rate of fetal umbilical cord around the neck those lower than 0.01kg/m2 per week was reduced by half approximately(P<0.05). The study found that energy intake in pregnancy and family income were positively related to weight and BMI growth rate during 5~17weeks of gestation(P<0.01), but duration of exercise or reaction of pregnancy were negative related to weight and BMI growth rate during 5~17 weeks of gestation(P<0.05).Conclusion: Weight and BMI growth rate in early and mid-pregnancy influence gestational weight gain and pregnancy outcomes. And it is effective to stay appropriate energy intake, relieve pregnancy reaction, and exercise in moderation for keeping appropriate weight gain in early and mid-pregnancy.
