The Clinical Analysis Of Lymph Node Metastasis Regularities In Hepatic Flexure Carcinoma

Objective:To explore the factors related with the lymph node metastasis in hepatic flexure carcinoma as a reference for lymph node dissection.Methods:This study is a retrospective analysis of 340 cases of surgical treatment of hepatic flexure carcinoma in the department of Colorectal Surgery in Fujian Medical University Union Hospital from January 2000 to December 2014. Lymph node metastasis rate =(node-positive cases / total cases) × 100%.Results:1. One hundred and seventy-five patients(51.4%)of the 340 included patients had lymph node metastasis:47.6%with N1 tumors,6.4% with N2 tumors,17.6% with N3 tumors(p <0.05).2.Univariate analysis showed that preoperative plasma CEA(carinoembryonic antigen)level, T stage(depth of tumor invasion), degree of tumor differentiation, histological type and pathology type were significantly correlated with the lymph node metastasis rate of hepatic flexure carcinoma(p <0.05);however,gender, tumor diameter, age, tumor location,and surgery way were not correlated with the rate of metastasis(p >0.05).3. No.223 lymph node positive rate in Hepatic flexure carcinoma revealed T-stage and poor differentiation(p <0.05); No.206 lymph node positive rate in Hepatic flexure carcinoma revealed T-stage and Tumor location(p <0.05)?Conclusion:1. Degree of tumor differentiation,Uerative/infiltrative morphology, T3-4stage and the preoperative plasma CEA( carinoembryonic antigen) level are significantly associated with the rate of lymph node metastasis of hepatic flexure carcinoma.2.The metastasis incidence of skip lymph node was happened in patients who was in degree of tumor differentiation and T3-4 stage.3. No.223 lymph node positive rate in Hepatic flexure carcinoma revealed T-stage and poor differentiation. No.206 lymph node positive rate in Hepatic flexure carcinoma revealed T-stage(T3+T4) and Tumor location( transverse).
