?Objective?Brain stem infarction is a rare type of cerebral infarction.The clinical manifestations of brain stem infarction are complicated,its mortality and disability rate are high,and its prognosis is poor,even those who survive are left with different degrees of neurological dysfunction,which cause heavy harm to human's health,and bring heavy burden and pain to the society and families.A research showed that diabetes mellitus(DM) is more frequently involved in the posterior cerebral artery than the anterior.Therefore,our research aimed to find the relationship between DM and brain stem infarction to provide strategy for early prevention and prompt treatment,and to reduce morbidity, mortality and disability rate by retrospectively review.?Methods?Date were retrospectively reviewed for 124 consecutive patients who were admitted with acute brain stem infarction to the neurology department at our hospital.Of these patients,DM was diagnosed in 49 and the others are non-DM.Analyze the clinical features,the systolic blood pressure,blood test results,carotid duplex ultrasound and brain magnetic resonance imaging(MRI) between these two groups.Analyze the risk factors of brain stem infarction and find the relationship between DM and brain stem infarction.All of the data were analyzed by SPSS19.0 statistical software,the results were compared with the t-test or ?2-test,and further determined whether the variables were statistically significant,and P<0.05 was considered statistically significant.The risk factors of brain stem infarction with DM were analyzed by multi-variate logistic regression analysis.?Results?There is no age,sex or history of hypertension disease differences between the DM and non-DM group.The systolic blood pressure,glycated haemoglobin(Hb A1c), fasting plasma glucose(FPG),TC,TG,LDL-C,apolipoprotein B(Apo B) in DM group were higher than those in non-DM group,and the HDL-C and apolipoprotein A1(Apo A1)/Apo B ratio in DM group were lower than those in non-DM group.from Multi-variate logistic regression analysis, glycated haemoglobin(Hb A1c) was the risk predictor of DM with Brain stem infarction,while Apo A1/Apo B ratio was the protective facror.?Conclusions?DM is more closely associated with brain stem infarction than the other cerebral infarctions.DM high systolic blood pressure,high TC,high TG and high Apo B are the risk factors of brain stem infarction, while HDL-C and Apo A1/Apo B ratio are the protective factors.Therefore,monitoring the blood glucose regularly,maintaining the blood glucose in normal level,reducing the intake of fatty substances,and using the lipid-lowering drugs properly can effectively prevent the occurrence of brain stem infarction.
