?Objective?Survey the infection of helicobacter pylori and the infection risk factor;to research the association of nucleotide polymorphism of mi RNA targe site with the different kind of gastric cancer; explore the interaction effects of inflammation factors related to mi RNA targe site related gene-gene and gene-environmental factors.?Methods?1. Gastric helicobacter pylori urease antibody detection kit(colloidal gold)method. A random sample about Xianyou county hospital gastroscopy and lived in xianyou county for more than 10 years, 35 years old in 190 cases. By gastroscope as the gold standard, rapid urease test under the condition of comparative gastric helicobacter pylori urease antibody detection kit(colloidal gold), authenticity and reliability; Randomly gastroscope inspectors in 84 cases, comprehensive comparison of colloidal gold method, ELISA and parallel test authenticity and reliability of colloidal gold method to detect the crowd feasibility of HP infection.2.The current situation of Hp infection and its risk factors. According to adjust gastric cancer mortality stratification, using three stages stratified cluster sampling method to extract the country population in 788 cases. Verify the colloidal gold method is feasible on the basis of detecting helicobacter pylori crowd, through the questionnaire survey to obtain the information such as the risk factors of helicobacter pylori infection. Integrated application of chi-square test and conditional Logistic regression analysis.3.1:1 matched case-control study method, collection of Xianyou county hospital in Fujian province by histopathology confirmed the new send a total of 313 cases of gastric cancer,During the same period by age, gender, the villages and towns matching the health people in Xianyou county as control. Collected through questionnaire survey and laboratory testing cases and compared the general situation,the environmental exposure risk factors and helicobacter pylori infection in peripheral blood of information, collection of cases and controls the object of study 5 ml blood samples. Gastric helicobacter pylori urease antibody detection kit(colloidal gold)analysis and evaluation: rapid urease test under gastroscope as gold standard, and the results showed that gastric helicobacter pylori urease antibody detection kit(colloidal gold) sensitivity of 90.82%, 68.48%, consistent rate was 80.00%, the Kappa value was 0.77; Compared with ELISA, the sensitivity was 90.32%, 90.32%, were higher than ELISA sensitivity(77.42%), specific(58.49%). Random selection of 10% winter samples and 10% summer samples consistent repetition rate 100.00%, 5% random blood samples from four batches colloidal gold test results consistent rate was100.00%.2.The risk factors of helicobacter pylori infection in investigation: xianyou adults HP positive rate was 47.2%. Male infection rate was 48.8%, the female HP infection rate was 45.0%, male HP infection rates than female; 55- and 65- age group HP infection rate is higher: 46.8%, 51.0%, the lowest ~ 75 age group HP infection rate: 41.9%. HP infection risk factor: raising chickens and ducks pigs and poultry, restaurant,sometimes or never use detergent and other lifestyle; History of HP infection protection factor: medication(non-steroidal anti-inflammatory drugs and antibiotics.The healthy people group strains of HP infection is given priority to with type II,gastric cancer cases are mainly composed of type I, difference is statistically significant.3.Inflammation factors related to the mi RNA target sequence and gastric cancer susceptibility analysis:IL10RB gene rs3171425 loci associated with the cardia gastric cancer susceptibility,AA genotype is a poor genotypes of cardia cancer occurrence, the risk of cardia cancer is carrying GG genotype 2.12 times(95% CI: 1.02 4.37); IL-1F5 gene rs2515402 loci associated with cardia cancer susceptibility, compared with wild type AA, CC happened cardia cancer suppressor gene type, OR a value of 0.32(95% CI:0.12 0.83).4.The interaction analysis of gene-gene /gene-environment and the joint action:In the cardia cancer, carrying rs3171425 AA genotype and pesticides age 18 or less population risk of the cardia cancer is to carry rs3171425 G allele but not 7.30 times,pesticide use(95% CI: 3.06 17.44); Carry rs3171425 AA genotype and people often eat hard food risk of the cardia cancer is to carry rs3171425 G allele and eat soft hard moderate population of 6.79 times(95% CI: 3.56 12.95). The multiplication effect analysis shows that carry rs3171425 AA genotype with pesticides, food hard there are18 years old or less multiplication interaction.?Conclusion?1.Gastric helicobacter pylori urease antibody detection kit(colloidal gold) is fast,simple, authenticity and high accuracy test method of HP infection, can be used in detection of HP infection.2. Xianyou adults HP positive rate is 47.2%; Healthy people group strains of HP infection is given priority to with type II, gastric cancer cases are mainly composed of type I. Xianyou HP infection risk factors for the public to be adults are: raising chickens, ducks, pigs and other poultry, always or offen eat in restaurant, occasionally or often do not use detergent lifestyle; Its protection factor is: a history of medication(non-steroidal anti-inflammatory drugs and antibiotics). HP infection intervention need to take further steps.3.The cardia gastric cancer and non-cardiac gastric cancer has different influence of inflammatory factors related the mi RNA target sequence polymorphism, including different susceptibility polymorphism loci:IL10RB gene loci rs3171425 of AA genotype is cardiac gastric cancer occurrence of bad genes, IL1F5 gene loci rs2515402 CC genotype is the protection of the cardia cancer genes;Gene-environment interaction is different, in the cardia cancer of the stomach, food hard, pesticide use age < 18 years old, the combined IL10 RB gene loci rs3171425 and positive intersect. The research in the cardia cancer has not been the best interaction model. Hint according to the different risk groups with specific guidance and provide prevention and control measures, can effectively achieve the goal of prevention of gastric cancer occurs.
