Objectives: To evaluate the effects of different trendelenburg position angles on the ventilatory and circulatory function for colorectal cancer laparoscopic surgery.Methods: 125 patients pathologically confirmed colorectal cancer patients undergoing elective resection of colorectal cancer(Dixon) were selected as the research objects. After the establishment of CO2 pneumoperitoneu, the patients were treated by Trendelenburg position with low head and high hip as a downward tilt to the bed. Then sequencely djustedthe posture angle to 20°,30°and 40°. Each position lasted 15 min. 10 min after the regulating of body position in each time(as named T1, T2, and T3 moment), the heart rate(HR), systolic blood pressure(SBP), diastolic blood pressure(DBP), mean arterial pressure(MAP), peak of airway pressure(Ppeak), pulse oxygen saturation(Sp O2), tidal carbon dioxide pressure(PETCO2) and the operative field exposure satisfaction were recorded. Then we analyzed the effects of different body posture angles on respiratory, circulatory function and the surgical field exposure.Results: Patients in T2 moment had statistically significantly increased SBP, MAP compared to T1 moment.At the T3 moment, patients' HR, SBP, DBP, and MAP were increased significantly compared with the T1 moment(P<0.05). At the T3 moment, patients' HR, SBP, DBP, and MAP were increased significantly compared with the T2 moment(P<0.05). Patients in T2 moment had statistically significantly increased Ppeak, PETCO2 compared to T1 moment.At the T3 moment, patients' Ppeak, PETCO2 were increased significantly compared with the T1 moment(P<0.05). At the T3 moment, patients' Ppeak, PETCO2 were increased significantly compared with the T2 moment(P<0.05). The operative field exposure satisfaction were better in T2 compared to T1, and T3 moment had most excellent sight compared T1 and T2.Conclusions: The hemodynamics and the ventilate function, the operative field exposure satisfaction were acceptable during pneumoperitoneum when positioning the patients on Trendelenburg position with 30°angle.
