Expression Of Th17 Related Cytokines In Patients With Dilated Cardiomyopathy

Objective The mechanisms of dilated cardiomyopathy(DCM)are largely unknown. Recent literatures indicate that dilated cardiomyopathy was due to abnormal of autoimmunity, especially abnormal of Th17 cells and their related inflammatory cytokines. We investigate the expression of Th17 related cytokines in patients with dilated cardiomyopathy, and explore their roles and significance in DCM pathogenesis, to provide a theoretical basis for exploring new diagnostic markers in dilated cardiomyopathy.Methods Thirty-three normal controls and a total of sixty-four patients with DCM who were diagnosed and treated in Xiamen cardiovascular hospital were observed from February 2014 to December 2015. The clinical data of cases that enrolled in our research were recorded, and blood samples were drawn. The supernatant was put in EP tub after centrifuged and stored at-80?. Expressions of 15 kinds of Th17 cytokines from plasma were detected by using Bio-Plex protein suspension array technology, including sCD40L, IL-4, IL-6, IL-17 A, IL-23, TNF-?, IL-33 and so on. Blood samples of 11 cases of DCM patients were respectively drawn in the acute stage(at admission) and in remission stage(at discharge). Dynamic changes of expression of Th17 related inflammatory factors in different periods were analyzed. Expression of inflammatory cytokines m RNA from lymphocytes were detected by Real-time PCR method.Results 1)Expression levels of Th17 related inflammatory factors from plasma in CON group VS DCM group respectively were(1)s CD40L( 251.60±32.96 pg/ml VS 725.00±51.07 pg/ml),(2)IL-4(26.03±7.13 pg/ml VS 62.52±6.85 pg/ml),(3)IL-6(10.12±2.68 pg/ml VS 66.31±9.16 pg/ml),(4)IL-17A(19.16±3.79 pg/ml VS 79.58±12.43 pg/ml),(5)IL-23(79.52±6.79 pg/ml VS 116.50±9.73 pg/ml),(6)TNF-? (10.95±2.18 pg/ml VS 19.49±3.66 pg/ml),(7)IL-33(272.70±34.49 pg/ml VS 237.10±24.89 pg/ml). Compared with control group at the same time point, levels of s CD40 L, IL-4, IL-6, IL-17 A, IL-23 obviously increased in DCM group(P<0.05). 2) Levels of s CD40 L in control group and patients with dilated cardiomyopathy in acute stage and in remission stage were 251.60±32.96pg/ml, 838.48±49.56pg/ml, and 623.34±67.21pg/ml respectively, and the differences are statistically significant(P<0.05). Other cytokines have no statistically changes in different stages. 3)TNF-?,IL-4, IL-6, IL-17 A m RNA in patients with dilated cardiomyopathy were higher than those of controls from lymphocytes which collected from peripheral blood.Conclusions Expressions of sCD40L, IL-4, IL-6, IL-17 A, IL-23 were significantly higher in patients with dilated cardiomyopathy. These Th17 related cytokines may play a role in the pathogenesis of dilated cardiomyopathy. sCD40L increased more, and it associated with disease severity. sCD40L may contribute to the assessment of DCM.
