Objective To develop teaching scheme with human patient simulator for critical nursing simulation teaching based on Jeffries Simulation Framework,and to evaluate the effectiveness of the teaching scheme by applying the program in teaching practice,then provide the theoretical and practical reference for critical nursing simulation teaching.Methods Part1 Development of teaching scheme with human patient simulator for critical nursing simulation teaching To design the draft teaching scheme with human patient simulator for criti cal nursing simulation teaching based on Jeffries Simulation Framework and lit erature review. After experts consultation,the teaching scheme was modified an d the teaching scheme was finally established. Part 2 The application of teaching scheme with human patient simulator for critical nursing simulation teaching This part used quasi-experimental method.Total 71 undergraduate nursing students from one college was enrolled in by convenience sample, and then divided into experiment group(37 students) and control group(34 students) according to the sequence of internship in intensive care unit(ICU) in one hospital in Shang Hai.After agreement,the experiment group received 3times,(4credits each time,total 12credits) critical nursing simulation training in 4 weeks' internship in intensive care unit, by using the ‘teaching scheme with human patient simulator for critical nursing simulation teaching',while the control group only received conventional teaching methods.The teaching objectives,contents of course,credits of internship and teachers arrangement were same.The evaluation indicators include score of knowledge and skills,comprehensive ability in clinical simulation scenario,behavior in high-fidelity simulation teaching and the evaluation of critical nursing simulation training. Part3 Students, experience of critical nursing simulation teaching using human patient simulator Qualitative research was used in this part.Part of students form experimen tal group were chosen to join the 1-hour semi-structured interview.The contents of the interview were transcribed to words,after repeated reading,then encoded,analyzed the data and classified,last extracted the theme.Result Part1 Development of teaching scheme with human patient simulator for critical nursing simulation teaching 1.The specialist authority coefficient in the development of the teaching scheme was 0.868. The variation coefficient of feasibility of the teaching scheme were all below 0.5. 2.The final teaching scheme includes teaching objectives,contents of course,program schedule,teachers arrangement and teaching evaluation.The program schedule was consisted of teaching module,specific teaching structure,time arrangement,contents and number of faculty. Part 2 The application of teaching scheme with human patient simulator for critical nursing simulation teaching 1.After critical nursing simulation teaching,there was no statistical difference between the two groups in scores of knowledge examination(p>0.05),whereas in skills assessment,the scores of experimental group were significantly higher than control group(p<0.01). 2. After critical nursing simulation teaching,the scores of knowledge examination and skills assessment of two groups were significantly higher than teaching before.(p<0.01). 3.After critical nursing simulation teaching, there was no statistical difference between the two groups in the total scores of assessment of implementation in simulation scenario(p>0.05).In the assessment of quality of implementation,there were no statistical differences between the two groups in emergency ability, observation,cooperation and nursing record(p>0.05),whereas the scores of communication ability of experimental group were significantly higher than control group(p<0.01). 4.After the first time of critical nursing simulation teaching,the total scores of comparative performance of students in experimental group were(4.12±0.45),and communication(4.17±0.55),cooperation(4.41±0.50),clinical decision(4.02±0.49),professional knowledge and skills(3.86±0.60),professional development and critical thinking(4.13±0.52).After the third time of critical nursing simulation teaching,the scores respectively were(4.38±0.43)?(4.44±0.47)?(4.60±0.45)?(4.28±0.49)?(4.18±0.52)?(4.40±0.45), the scores of 5 dimensions and total score of the third time were significantly higher than the first time(p<0.05 or p<0.01). 5.The result of critical nursing simulation training evaluation showed that satisf action rate of total effect of experiment group students was 100%. And the sat isfaction rate of preparation before teaching was 91.9%, contents of training an d teachers' ability was 94.6%.And other aspects,such as objectives,practicability and designability of contents, simulation scenario and time arrangement was 97.3%.Part3 Students, experience of critical nursing simulation teaching using human patient simulator The students, experience from critical nursing simulation teaching include the experience of teaching process,the experience of teaching effect and the clinical role consciousness,total 3 themes.Conclusion 1.The teaching scheme designed in this study was rigorous,the objective was clear, can meet the requirements of critical care nursing practice.The contens were feasible and practical,the time arrangement and form of this teaching scheme match the rquirement of simulation teaching and can be applied to critical nursing simulation training. 2.There was quite guidance and operability of the teaching scheme with human patient simulator for critical nursing simulation teaching,and can be applied to critical nursing simulation training,and to apply the advantages of intelligent of human patient simulator in clinical teaching. 3. To some extent, the teaching scheme can enhance knowledge and improve skills of nursing students,meanwhile can enhance their comprehensive ability and most of them felt satisfied with the teaching scheme. 4. The qualitatively study showed the high-fidelity simulation teaching can active learning atmosphere, increase students learning enthusiasm and guarantee the teaching effect, enrich the connotation of the nurse role.Meanwhile,the students have some suggestion on simulation scenario and teachers, role. So we should still continue to improve the teaching method in the future, in order to maximize the advantages and value of the human patient simulator.
