Association Between EGFR/ALK Mutation And Clinical Characteristics Of Early-middle Stage NSCLC Patients After Surgical Resection

[Objective] To explore the association between the epidermal growth factor receptor(EGFR)mutation, anaplasticlymphoma kinase(ALK)mutation and the clinical characteristics,histologic subtypes of non-small-cell-lung-cancer(NSCLC).And try to find the association between EGFR and ALK mutation.[Methods] In this study,we reviewed specimens of non-small-cell-lung-cancer(NSCLC)from 2014.9 to 2015.9 in WuHan Union Hospital. The association of EGFR and ALK mutation and clinical characteristics was analyzed by SPSS22.0.[Results] EGFR mutation rate in women(69.7%)was higher than in men(43.6%)(P<0.05). Regarding histologic subtypes of adenocarcinoma,mutation was more observed in lepidic predominant(83.3%) and papillary predominant(73.1%)adenocarcinoma. The mutation rate had no significant difference regarding the size of tumor;the rate of lymph nodes metastases is higher in mutation group(36.3%)than in wild group(29.7%).The expression of TTF-1 was likely to lead to a higher EGFR mutation,but with no significant difference(P=0.05),while NapsinA do lead to a higher EGFR mutation(P<0.05).In this study,we only got 21 ALK(D5F3)(+) specimens(5.64%),more seen in female?younger adenocarcinoma. Besides,mutation was more observed in solid predominant adenocarcinoma and/or with mucus.[Conclusion] The mutation rate of EGFR was 55.5%?Women,non-smokers,histologic subtypes of adenocarcinoma with the presence of nonmucinous BAC component and papillary component were found to be significantly associated with a higher incidence of EGFR mutation,mixed mucinous BAC components had a lower EGFR mutation.EGFR mutation was also higher in adenocarcinoma express TTF-1 and NapsinA.The rate of lymph nodes metastases is higher in mutation group,which means the wild group obtains better DFS than mutation group. The mutation rate of ALK was 5.64%,more in female?younger patients,and more in subtype of solid predominant adenocarcinoma and/or with mucus.
