Objective: To investigate the clinical characteristics and histopathological features of idiopathic membranous nephropathy(IMN) in young people.Methods: Patients had been clinically and histologically diagnosed with IMN at the First Affiliated Hospital of Fujian Medical University from January 2006 to December 2015 were included and divided into two groups for ?44 years, and ?44 years.Results: 1. 147 patients had been diagnosed with IMN, accounting for 13.12% of patients who accepted renal biopsy in our hospital over 2006-2015.(528 cases in 2006-2010, 592 cases in 2011-2015), accounting for 18.01% of primary glomerular disease. 28 IMN patients in 2006-2010, accounting for 5.30% of the same period,119 IMN patients in 2011-2015, accounting for 20.10% of the same period. 2. There were 60 young patients and 87 elder patients. Compared with elder group, there were low incidence rates of hypertension, renal failure, hypercholesteremia in the youth group. There were also significant statistical differences among BMI, GLU, Scr, CYS-C and GFR in two groups. As for youth group, DBP, RBC, HB, UA, TC, TG, LDL, TB, ALB, Scr, Ig G and Ig M were also significant statistical differences between males and females. At the mean time, females of elder group had significant statistical differences in DBP, HB, BUN, SCR, APOA, UA and HDL from males in elder group. 3. Most cases were classified as stage I or II membranous lesions in pathology, accounted for 91.84%. Compared with elder group, there were low incidence rates in glomerulosclerosis and tubule-interstitial pathological changes in youth group. About Immunofluorescence staining, Ig M, C3 and C1 q deposition ratio in the two groups had no significant difference. In stage I, the incidence of hypertension, hypercholesterolemia, cys-c, the level of glomerular filtration rate and rate of arteriolehyaline degeneration in youth was lower than elder. In stage ?, youth group was in a low incidence of kidney failure, high blood pressure, fasting glucose, red blood cell count, hemoglobin content, cys-c, serum creatinine, glomerular filtration rate than elder group. In stage ?, only the incidence of renal failure and level of BUN in youth group lower than the elder group.Conclusion:In the last five years, the frequency of IMN was increased Significantly. The clinical characteristics and histopathological features of IMN differ in youth and elder groups. the proportion of hypertention, GFR<90ml/(min.1.73m2), hypercholesteremia, glomerulosclerosis, tubulointerstitial lesions in youth group were less severe than elder group, especially young females.
