Effect Evaluation Of Multifunctional Conservative Neck Dissection In Thyroid Carcinoma

Objective: Thyroid carcinoma is the high incidence of head and neck cancer, and lateral cervical lymph node metastasis often occur. Cervical lymph nodes dissection of thyroid carcinoma with cervical lymph nodes metastasis(c N1b) in patients is the most effective treatment, also has been recognized as the most important surgery. And neck dissection surgery will be most of the neck of the sensory nerves and other vascular resection of both often appear postoperative swelling of the face, permanent ear lobe, head, neck and shoulders skin numbness. This study compared with modified radical neck dissection and effect of multifunctional neck dissection with preservation operation and postoperative recovery, investigate its clinical utility.Method: For clinical suspected lateral neck lymph nodes transfer of thyroid cancer(c N1b) patients, according to tumor original sent stove size, and tumor local infiltration degree, and lateral neck lymph nodes of transfer degree, respectively line improved type neck cleaning operation(also called functional neck dissection) and multifunctional conservative neck dissection, through recalled analysis 167 cases patients of clinical information, on retained neck outside vein, and neck bundle, and neck cross moving vein of multifunctional conservative neck dissection of radical effect and operation function for assessment, Observation of postoperative complication and recurrence rates.Results: Compared with traditional improved type neck clear operation, retained external jugular vein, and neck bundle, and neck cross moving vein of multifunctional conservative neck dissection patients, its ear department, and neck shoulder skin feel basically in operation after 3-6 months within recovery normal, facial department and supirior neck swelling of occurred rate obviously reduced; and after with diagnosis, compared two group patients of local lymph nodes recurrence transfer and body distance transfer of situation, two group recurrence rate differences no significantly.Conclusion: Multifunctional conservative neck dissection, not increased too more surgery time, and not increased surgery difficult degrees; in surgery resection of completely, and the number of operated lymph nodes, and functional neck cleaning are not obviously different; but, obviously improved has neck cleaning operation patients earlobe district, and supirior neck shoulder skin feel of recovery, and, due to neck shallow vein of retained, patients after operation facial department and the supirior neck paragraph swelling of occurred rate obviously reduced. Therefore, preserving the external jugular vein, carotid plexus, transverse cervical artery and vein of multifunctional retention of neck dissection in the treatment of thyroid cancer is a more reasonable operation, this technique improves the quality of life of postoperative patients and has extended clinical value.
