Objective: By constructing a 3D model of aneurysm, fully display the anatomic features of the patients with intracranial vascular, important blood vessels, nerves and aneurysm anatomic relationship, design of aneurysm neck separation scheme, selection of suitable aneurysm clips, reduction in the process of surgical treatment of unnecessary exposure of anatomic structures, reduce the surgical trauma, for the diagnosis and treatment of aneurysm provide guidance and improve the surgical success rate and reduce the incidence of complications of prediction of before operation.Methods: Application of randomized controlled study on the method of selection of hunt Hess score the same 20 cases of intracranial aneurysm patients and 10 cases of patients before operation for aneurysm model 3D printing, and complete the operation before clipping simulation, and then the model was applied to intraoperative surgical guidance, control group(10 cases) by routine application of CTA and DSA imaging data for surgery, in aneurysm clipping completed application intraoperative Doppler, nerve endoscopic exploration flow situation. Analysis the clipping, clipping after recanalization, excessive clamping closed and the incidence of postoperative infarction.Result: 3D model to guide the operation of group 10 cases of patients with intracranial artery were according to the preoperative simulation to be completely clipped, clip closed after intraoperative ultrasound, endoscopic exploration residue, stenosis; traditional surgery group 10 cases of patients with intracranial aneurysm clip closed after the completion of the intraoperative ultrasound, endoscopic exploration survey showed 8 aneurysms were completely clipped, 1 case of clipping is given again adjust the aneurysm clip, no parent artery stenosis and perforating vessel occlusion, 1 case of aneurysm patients before surgery to rupture.Conclusions: Simulation guide aneurysm clipping to reduce artery aneurysm rupture bleeding risk and postoperative infarction is important, significantly reduce artery aneurysm clipping operation error in the process of clip, clip and excessive clamping closed the incidence of constructing 3D model of intracranial aneurysm, and improve the safety of the operation, effectively improve the prognosis of patients.
