Objective SP gender differences are significant. Clinical records analysis and laboratory tests were used to investigate the clinical characteristics of female with spontaneous pneumothorax, and to provide a reference guide for the clinical individual treatment for female SP.Methods The clinical data of 834 cases (male 738, female 96) from Nanjing Chest Hospital of Southeast University were analyzed retrospectively. Review and analysis of the patients with the age of onset, past history, focusing on familial pneumothorax disease, skin lesions, renal tumor, chronic obstruction of pulmonary tuberculosis, pulmonary bulla side, location, size, shape, exploratory laparotomy see distribution of pulmonary bullae, postoperative catheter time, postoperative hospital stay, the FLCN gene mutation. The expression intensity of estrogen receptor, progesterone receptor and CD 10 in lung tissue were evaluated by immunohistochemical staining and immunohistochemical staining in 34 cases of female SP patients. Of 217 patients (192 males and 25 females) with family history or highly suspected BHD syndrome, FLCN gene mutations were detected in the peripheral blood of patients.Results?The age of onset of female patients had two peaks, respectively, in 15-30 and 40-60 years old. There was no difference in mean age between genders (P>0.05).? The onset age of PSP group in female patients was significantly lower than that in patients with SSP group (P<0.05).?There was no difference between the PSP group and the SSP group (P>0.05).?The PSP group of postoperative hospital stay was significantly shorter in the SSP group (P<0.05).?The PSP group located in the apical bullae proportion was higher than that of group SSP (P<0.05).?SP in the female patients with pulmonary bulla resection specimens, the positive rate of ER was 97.06%(33/34), PR positive rate was 67.65%(23/34) and positive rate of CD10 was 70.59%(24/34).?The FLCN gene mutation rate of female was higher than male (P< 0.05).Conclusion?For spontaneous pneumothorax in female of childbearing age should be taking into account the possibility of CPTX, and on the basis of the clinical features, especially with auxiliary examination and endoscopic exploration, pathological examination and immunohistochemistry of identification, early diagnosis, to be a reasonable treatment. ?The mutation rate of FLCN gene in female patients was significantly higher than that in male patients, and mutation of FLCN may be one of the main pathogenic factors of female PSP.
