The Research Of The Preoperative Diagnosis In The Anal Fistula By Three-dimensional Transrectal Ultrasound

Objectives Through by three-dimensional transrectal ultrasound diagnostic technology,we can research the application in the preoperative diagnosis of anal fistula.Methods Selection during September 2015 to January 2016 in Henan University of Chinese medicine 40 cases,the third affiliated hospital of complex anal fistula patients were randomly divided into three-dimensional transrectal ultrasound inspection group and multi-slice spiral CT inspection group.With the results as the final diagnosis standard,for 3D-TRUS group and MSCT inspection group preoperative diagnosis results were compared and analyzed.Results By t test,the patient's ages,course of the disease,preoperative CRP,PCT,the WBC,neutrophils,were no statistical difference(P>0.05)between 3D-TRUS inspection group and MSCT inspection group.By chi-square test,the sex of the patients between 3D-TRUS group and MSCT inspection group had no statistical difference(P>0.05).Preoperative diagnosis of anal fistula in the mouth: compared with surgical results,3D-TRUS diagnosed 19 cases existing mouth,MSCT diagnosed 14 cases existing in the mouth.By chi-square test,two groups in the anal fistula mouth diagnosis had statistical difference(P<0.05).3D-TRUS inspection group within the preoperative anal fistula mouth diagnosis accuracy was higher than MSCT inspection group,mouth inside the anal fistula preoperative diagnosis with higher preoperative diagnosis basis.Preoperative diagnosis of anal fistula with fistula: compared with surgical results,3D-TRUS preoperative diagnosis of presence of 15 cases of fistula,confirmed that the presence of 17 cases of fistula,postoperative diagnosis accuracy rate was 88.23%;MSCT preoperative diagnose the presence of 8 cases of fistula,confirmed that the presence of 15 cases of fistula,postoperative diagnosis accuracy rate was53.33%.By chi-square test,the two groups in preoperative diagnosis of fistula had statistical difference(P<0.05).By 3D-TRUS could accurately diagnose the narrow thin fistula more.Preoperative diagnosis of anal fistula type: compared with surgical results,through the chi-square test,there was statistical difference between 3D-TRUS inspection group and MSCT inspection group in the preoperative diagnosis of anal fistula type.3D-TRUS preoperative diagnosis had advantages of low complex anal fistula,and MSCT preoperative diagnosis had advantages of high complex anal fistula.Two different methods of examination in the diagnosis of anal fistula type had its advantages.Conclusion The three-dimensional transrectal ultrasound not only has high value in the diagnosis of the anal fistula mouth,but also can observe and analysis the anal fistula from the sagittal,coronal and transverse views,through the three-dimensional imaging.It can clearly display anal fistula contorts,have clear without a fistula and also crissum the relationship between the internal and external sphincter.The three-dimensional transrectal ultrasound has high value in the diagnosis of anal fistula,improves the accuracy of preoperative diagnosis and the strong ultrasonic diagnosis for surgery,to a certain extent,reduces the recurrence rate of anal fistula.The operation of three-dimensional ransrectal ultrasound is simple in.It has low cost,causes little pain to the patients,and also can get better in clinical popularization and application.
