Objects: By observeing the influence of manipulation of tendon-regulation of Orthopaedics and Trauma of Traditional Chinese Medicine to pain threshold in inflammatory pain rats evoked by injection of complete Freund's adjuvant and the adenosine content after topical manipulation treatment,to explore the analgesic effect of topical manipulation treatment on inflammatory pain and its mechanism of analgesic effect,thus to provide a theoretical basis for local analgesic effect of clinical manipulation of tendon-regulation.Methods: 24 healthy female SD rats were randomly divided into blank group(A1),Freund's adjuvant model group(A2),manual therapy group(A3),8 in each group.The blank group were not modeled,not given topical manipulation treatment,just fixed the same time using the same method with the manual therapy group for a total of 20 min at the beginning of the experiment.The Freund's adjuvant model group were injected 0.1ml complete Freund's adjuvant subcutaneously into the right paw on the first day of the experiment,to induce rats model of inflammatory pain,but were not treated,other processes were the same as the blank group.The A3 manual therapy group were injected subcutaneously 0.1ml complete Freund's adjuvant in the right paw of rats on the first day of the experiment,to induce rat model of adjuvant inflammatory pain,and were given topical manipulation treatment on the 4th day after the injectiont,digital pressing massage by middle finger which is commonly used was selected to treat the rats.In a quiet environment,the rat were fixed in a special Rat fixator,all keeping awake,and then were treated by digital pressing massage on Yongquan point for 20 mins,after the treatment,pain threshold caused by hot board was measured and recorded,and the rats were killed and the plantar muscle tissue were removed for examination.To observe changes in pain threshold by measuring the Thermal paw withdrawal reflex,and evaluate rats model of inflammatory pain by measuring foot circumferenceto assess the feet swelling degrees of rats.HWS-26 Electro-Thermostatic water bath was used to measure thermal pain threshold of rats respectively at three time points(1day before modeling,4 days after modeling which is before treatment,after treatment)and pathological section was made and the content of adenosine was detected by ELISA kit.Results:1.General condition of rats: the blank group rats showed that fur was sleek,the mental state,activity,reaction and appetite of rats were normal.Rats of Freund's model group A2 and manual therapy group A3 after the modeling showed that rat inflammatory parapodum appeared red and swollen,peaked at around 24 h,they were characterized by fur losed luster,depression,less activity,and inflammatory side of the foot dared not touch the ground with claudication,which last for 3 days and then began to ease gradually.2.The difference of the rats paw circumference was not statistically significant compared with each other before modeling(P>0.05),model rat paw circumference increased significantly after modeling,peaked at about 24 h.Compared with the contralateral paw,there was a significant difference(P<0.05),and there was a significant difference(P<0.05)compared with the blank group.3.Pathological results: the muscle tissue in model rats showed muscle fibers atrophy,muscle fibers gap widened,inflammatory cell infiltration,proliferation of small blood vessels,dilatation and congestion,etc.Compared to the blank group,there were significant differences.4.The difference of the rats thermal pain threshold was not statistically significant compared with each group before modeling(P>0.05).thermal pain threshold of Freund model group A2 and manual therapy group A3 decreased obviously after modeling,there was a significant difference compared with the blank group(P<0.05).Thermal pain threshold of manual therapy group A3 increased significantly after treatment,with marked difference as compared with that before the treatment(P<0.05).Compared with each other after treatment,there was a significant difference of thermal pain threshold between the blank group A1 and other groups(P<0.05),There was a significant difference of thermal pain threshold in Freund model group A2 and manual therapy group A3(P<0.05).There was no statistical significance(P> 0.05)of Adenosine concentration in blank group A1 compared with Freund model group A2.Adenosine concentration in peripheral tissues of manual therapy group A3 increased obviously,there was a significant difference compared with the blank group(P<0.05),and there was also a significant difference compared with the Freund model group A2(P<0.05).Conclusion:1.As an antigen complex,complete Freund's adjuvant can make injection site produce redness,swelling and heat hyperalgesia behavioral responses,cause changes in histopathology,which is usually used to induce inflammatory pain model.2.The manipulation of tendon-regulation has remarkable local analgesic effect,can significantly improve the pain threshold of rats with inflammatory pain.3.The treatment of manipulation of tendon-regulation can increase the concentration of local neurotransmitter adenosine in the pain rats,and the local analgesic effect of manipulation of tendon-regulation may be played by the local release of adenosine.
