| Background and Objective : Proteinuria is one of the criteria for the clinical development of renal allograft injury,and it is also an independent risk factor for the progression of chronic renal allograft nephropathy,control and reduction of proteinuria after renal transplantation is the key to prevent renal allograft nephropathy and chronic allograft nephropathy,and it can significantly delay the progression of renal graft function damage.At Present,the prevention and treatment of proteinuria after kidney transplantation is mainly restricted by protein intake,ACEI,ARB and other drugs,such as the replacement of immunosuppressive agents,but the effect is not ideal.The appearance of proteinuria after kidney transplantation is mainly caused by the cross influence of many factors,such as immunity and non immunity.In recent years,the clinical and experimental studies show that the centella asiatica has the function of resisting the glomerular sclerosis,protecting the renal tubular and regulating the immune status and so on,because of this,the application of traditional Chinese medicine of snow the grass combined with immunosuppressive therapy after renal transplantation proteinuria,evaluate the efficacy and safety of the comprehensive program of centella asiatica combined with immunosuppressive treatment of postoperative urinary protein,to form a set with Chinese characteristics of low cost,safe and effective,easy to grasp the optimal treatment plan,which can be used widely,the social and economic benefits the harvest.Methods : To collect 60 cases of proteinuria after renal transplantation in Department of Urology,the First Affiliated Hospital of Henan University of traditional Chinese medicine,according to the inclusion and exclusion criteria strictly selected cases were randomly divided into treatment group and control group,two groups were based on CNI regimen(FK506 + MMF + Pred immunosuppression maintain plan)conventional treatment,the treatment group added Centella asiatica(30 g /d,with water),the control group did not take Centella asiatica.Respectively in the 1 month,3months,6 months determination patients,24 h urinary protein,and determined in urineof patients with qualitative analysis of protein,urine microglobulin,urine beta2-microglobulin,urine microalbumin,valley alanine aminotransferase,blood urea nitrogen,blood creatinine,total cholesterol),triglyceride(TG),bilirubin the in the June,changes in the index of the two groups were statisticallyResults: There is no significant difference in age,sex and medication between the two groups of patients,no statistical significance(P>0.05),the treatment group and compared to the control group,the treatment group the total efficiency of 86.67%,the total efficiency of 63.33%;control group,was statistically significant(p<0.05);after treatment,24 h urinary protein,urinary protein,urinary 1-microspheres alpha beta 2-microglobulin,urinary albumin,serum creatinine,total cholesterol,were significantly different than that before treatment,there was statistical significance(p<0.05),the treatment group compared with the control group,there was significant difference,statistically significant(p<0.05)before and after;alt,bilirubin,urea nitrogen,triglyceride and other indicators of the treatment in the two groups had no significant difference,no statistical significance(p>0.05).Conclusion: Centella asiatica combined with immunosuppressive agents can significantly decrease urinary protein,especially in the urine micro albumin;meanwhile,it also play an important role in reducing creatinine,improving the prognosis of transplantaed renal,prolonging the microcirculation of transplantaed renal,reducing blood cholesterol levels,increasing the blood perfusion,improving the chronic ischemic lesions;Centella asiatica as pure traditional chinese medicine preparation after renal transplantation in the treatment of proteinuria curative effect is satisfactory.there is no significant side effects and adverse reactions in it. |
