Objective:To well understand the application of imaging characteristics of spectral CT for hepatocellular carcinoma with begin or malignant portal vein thrombosis by using spectral CT and quantitative measurement such as spectrum energy curve, so that we can evaluate the potential of spectral CT in distinguishing begin or malignant portal vein thrombosis.Materials and Methods:34 patients with hepatocellular carcinoma with begin or malignant portal vein thrombosis in Qian Foshan Hospital from 2013.01 to 2014.12 were collected. All cases with cirrhosis had been proved by surgery or biopsy pathology. According to the operative and pathological findings, 34 cases were devided into portal tumor thrombus group(20 cases)and portal vein thrombosis(14 cases). Patients were examined by GE Discovery CT 750 HD, including plain scan, arterial phase, portal venous phase and lag phase enhancement scanning. Collimation : The enhancement scanning was triggered by Smart Prep. We started scanning after 8 s when the threshold was up to 100 HU. When the arterial phase scan was over, the portal phase venous was conducted after 30 s and the delay phase scan was conducted after 80 s.The scan scope was from the diaphragmatic top to iliac spines.Measure CT value of different Kev(40~140, 10 interval)of primary hepatic tumors and portal venous emboli and calculate the corresponding slope of spectrum energy curve. Statistic analysis relied on SPSS 19.0 software. When the statistic result showed P < 0.05, it was considered significant difference in statistical.Results:The result of spectrum energy curve and CT value in 40 Kevindicate that the primary hepatic carcinoma and portal tumor thrombus group was similar(P>0.05), but there was significant difference between primary hepatic carcinoma and portal vein thrombosis(P<0.05).Conclusion:Spectral CT can give us lots of information of primary hepatic carcinoma and portal thrombus by using spectrum energy curve and CT value in 40 Kev, which can distinguish portal venous emboli on a pathological level. This program will provide lots of theoretical basis for prognosis evaluation and clinical treatment of portal venous emboli.
