?Objective?: To investigate the changes in fractional anisotrophy diffusion in PCI and to determine correlation of the changes in fractional anisotrophy diffusion with acute Glasgow coma scale(GCS).?Methods?: 20 healthy adult volunteers(10 men, 10 women; age range,20—59 years) as the healthy control subjects were studied using magnetic resonance imaging(MRI) and diffusion tensor imaging(DTI).The FA values were measured in the different parts of the brain. The ROIs were superior longitudinal fasciculus,internal capsule, gems of corpus callosum, splenium of corpus callosum. 30 patients as the experimental group,according to the GCS into mild trauma group,medium injury group and severe trauma group,were evaluated by computer tomograpy(CT),magnetic resonance imaging(MRI) and diffusion tensor imaging(DTI).Fractional anisotropy(FA) values were measured at multiple locations and correlated with GCS. Results were compared with those of 20 healthy control subjects.?Results?1. The brain white matter fiber FA values of the same parts has no significant difference between bilateral hemispheres(P > 0.05), the brain white matter fiber FA values of different anatomical parts from this study was significant difference,between the ROIs of this study(P < 0.05). The FA value of the splenium of the corpus callosum is highest, and the white matter fiber of superior longitudinal fasciculus is the lowest FA value.2.In experimental, patients of brain injury with splerium of corpus callosum,gems of corpus callosum, fasciculus longitudinalis superior cerebrita between bilateral hemispheres, the value of FA, the difference is statistically significant(P < 0.05),differences of internal capsule parts FA value between bilateral hemispheres has no statistical significance(P > 0.05)3.The ROIs FA value in mild trauma group comparison with control group, there was no statistically significant difference(P > 0.05).In medium injury group, the FA value left side of wite matter were lower than control group, the difference is statistically significant(P < 0.05).In severe trauma group, FA values from splerium of corpus callosum,gems of corpus callosum, left side of the internal capsule and the left side of the longitudinal fasciculus were lower than the control group, the difference is statistically significant(P< 0.05), FA values both of the right side of the internal capsule and the right side of the longitudinal fasciculus area comparison with control group, there was no statistically significant difference(P > 0.05).4. Compared with the outcome of the first time of DTI examination,the FA values of the lesion area of second examination were significantly reduced in heavy group.5.FA values of splerium of corpus callosum,gems of corpus callosum, the left side of fasciculus longitudinalis superior cerebrita were significantly correlated with GCS in acute stage(P<0.05). FA values of right side of fasciculus longitudinalis superior cerebrita, internal capsule, were not significantly correlated with GCS in acute stage(P > 0.05).?Conclusion?1. DTI technology is a kind of noninvasive detection to track the integrity of brain white matter fiber and make the outcome accurate,objective and effective.2. The sensitive degree of DTI in the diagnosis of DAI is better than CT and plain MRI imaging technology.3. DTI technique can provide objective, accurate, effective and timely data to evaluate the condition of patients PCI and the analysis of the curative effect of treatment.
