Morphological Study On Protection Effect Of “Jiangyabao” Against Target Organ Damage In SHR

Background “Jiangyabao” series preparations were studied on the basis of "symptom" concept in Chinese medicine by key hypertension specialty in Henan Academy of Chinese Medicine of traditional Chinese medicine hospital.“Jiangyabao” were designed for essential hypertensive patients with different TCM syndrome types and developed into traditional Chinese medicine compound pharmaceutical preparations in hospitals.Long-term and multi-level clinical trials showed that it was synergistic to commonly used blood pressure medicine and had sustained buck,high rate of remission low adverse reaction and other features.Otherwise,it also had lipid-lowering and target-organ protection effect preliminarily.Previous animal researches indicated that(1)“Jiangyabao” blue tablet had some antihypertensive effect on spontaneously hypertensive rats(SHR),but significantly lower than the Enalaprila group.“Jiangyabao” green tablets and yellow tablets had no significant effect on blood pressure.(2)“Jiangyabao” series preparations could significantly improve the SHR target organ demage and reconstruct blood vessels,heart and kidney at different degrees.(3)“Jiangyabao” series preparations could improve and reverse the SHR blood vessels,remodel kidney and heart.And its mechanism might be associated with Organize local ACE-Ang?-AT1 R shaft and ACE2-Ang-(1-7)-Mas shaft balance and arachidonic acid metabolism(TXB2/6-Keto-PGF1a)regulation.SHR were chosen as experimental subjects to observe the pathological morphology effect in high blood pressure,blood vessels,the left ventricular myocardial and renal tissue of “Jiang yabao” series preparations(blue tablets,yellow tablets and green yellow tablets)Objective Study and compare the pathological changes improvement and reverse effect of “Jiangyabao” series preparations on SHR target organ,and provide experimental basis for clinical application.MethodWith its genetic parent-wistar Kyoto rats(WKY)were compared,Choose 12 weeks of spontaneously hypertensive rats(SHR),Divided into WKY group,SHR group,enalaprila group,Jiang yabao blue tablets,green tablets and yellow tablets group.Under the conditions of anesthesia in rats,we get the aorta,left ventricle,and kidney.(1)HE-staining,Observation and determination of media thickness in thoracic aorta(MT),cardiomyocytes transection diameter(CTD),glomerulosclerosis index(GSI)and the pathological morphological changes with the optical microscope combined with Leica QWin V3 pathological image analysis system.(2)Masson-staining,Observation and determination of the thoracic aorta,left ventricular,intramyocardial small arteries,Kidney interstitial collagen changes and collagen volume fraction(CVF),perivascular circumferential area(PVCA)are calculated respectively with the optical microscope combined with Leica QWin V3 pathological image analysis system.(3)Gomori's fuchsin-staining,Observation and determination of membrane elastic fiber changes in thoracic aorta,and elastin volume fraction(EVF)with the optical microscope combined with Leica QWin V3 pathological image analysis system.ResultCompared with WKY blank control group,aortic tunica media thickened obviously(P<0.01),vascular smooth muscle cells(VSMC)deranged,the nucleus number per unit area increased significantly,and part of inner membrane and outer membrane inflammatory cells showed infiltration phenomenon;Elastic membrane deranged,loosen,uneven even fractured,elastic fibers decreased significantly,EVF significantly reduced(P<0.01);there were a lot of collagen deposition in deep stained,CVF significantly risen(P<0.01)in SHR groups.“Jiangyabao” blue tabletls,yellow tablets,green tablets and Enalapril could obviously reduce CTD,CVF and EVF(P<0.05~0.01),and significantly reduce aortic pathological change.Compared with WKY blank control group,myocardial cells swelling degeneration,disarranged,broken,nuclei hyperchromatic and conentrated,cytoplasmic stainning deepened,perpendicular grain structure was not clear,and inflammatory cells showed no infiltration phenomenon,CTD significantly increased(P<0.01);there were a lot of collagen deposition in deep stained in myocardial interstitium and around small arteries,CVF and PVCA significantly increased(P<0.01)in SHR groups “Jiangyabao” blue tablets,yellow tablets,green tablets and Enalapril could obviously reduce CTD,CVF and PVCA(P<0.05~0.01),and improve cardiomyopathy rational change in different degrees.Compared with WKY blank control group,part of glomerular volume shrinken and focal sclerosis,renal tubular cloudy and swelling,basement membrane thickened,occasional protein type or red blood cells,renal interstitial showed congestive,inflammatory cells showed infiltration phenomenon and interstitial fibrosis;there were a lot of collagen deposition in deep stained in the glomeruli and interstitial,CVF significantly increased(P<0.01)in SHR groups.Kidney pathological changes were reduced in “Jiangyabao” blue tablets,yellow tablets,green tablets and Enalapril group significantly,GSI and CVF were significantly lower than SHR control group(P<0.01).Conclusion1.Obvious,target-organ damage,and pathological changes of morphology and structure in artery,left ventricular and renal tissue appeared in 24-week-age SHR.2.SHR blood vessels,heart and kidney tissue damage and reconstruction induced by high blood pressure might be associated with interstitial collagen deposition and reduction of elastic fibers in tunica media of artery.3.“Jiangyabao” series preparations could improve and reverse SHR blood vessels,myocardial and renal tissue damage and reconstruction.Its mechanism was partly associated with reduction of collagen deposition,rise of membrane elastic fibers in the tunica media of artery.
