Clinical Analysis Between Supratentorial Cerebral Watershed Infarction And TCM Constitution

Objective: To explore the characteristics of TCM Constitution distribution in the patients with supratentorial cerebral watershed infarction and the relationship between the lesion classification of supratentorial cerebral watershed infarction, traditional Chinese Medicine Constitution and the large vessel stenosis of the internal carotid artery system.Method: We collected general information and clinical data of 127 patients with supratentorial cerebral watershed infarction confirmed by cranial CT or MRI and clinical diagnosis. We evaluated the TCM Constitution, analyzed the lesion classification of supratentorial cerebral watershed infarction and the lesion vessels and stenosis degree of the internal carotid artery system. Finally all the data are analyzed by SPSS 19.0.Result:(1) The percentage of qi deficiency, phlegm dampness and yang deficiency accounts for 21.26%, 16.54%, 14.96%.(2) The age distribution of male and female in cerebral watershed infarction is statistically significant. Greater than or equal to 60 year old patients have 90 cases, accounting for 70.87%.(3) The study has shown that there was no significant difference in the incidence of large vessel stenosis of the carotid artery system between different constitutions. The study has shown that the differences of constitutions are not statistically significant in the distribution of the large vessel stenosis of the internal carotid artery system.(4) Upper cortex, Upper part of the lower part of the cortex, postpallium are most common lesion types of supratentorial cerebral watershed infarction. The mixed type is more than the cortical type and subcortical type.(5) The percentage of internal carotid artery, middle cerebral artery and anterior cerebral artery accounts for 41.50%(61/147), 48.98%(72/147), 9.52%(14/147).(6) The incidence rate of severe stenosis or occlusion of middle cerebral arteries and internal carotid arteries was statistically significant.(7) Different focal types of supratentorial cerebral watershed infarction about the distribution of vascular stenosis of the internal carotid artery system have statistical significance.(8) The percentage of qi deficiency, phlegm dampness and yang deficiency accounts for 20.18%, 16.51%, 15.60%. There is no significant difference in the degree of vascular stenosis between different constitutions.(9) There are statistically differences in the degree of vascular stenosis between different types of lesions,Conclusion:(1) Qi deficiency, phlegm dampness and yang deficiency are the high risk of supratentorial cerebral watershed infarction.(2) The supratentorial cerebral watershed infarction is more common in the elderly. The onset age of male is lower than that of female.(3) The study has shown that there was no significant difference in the incidence of large vessel stenosis of the carotid artery system and the distribution of the large vessel stenosis of the internal carotid artery system between different constitutions.(4) Upper cortex, Upper part of the lower part of the cortex, postpallium are most common lesion types of supratentorial cerebral watershed infarction. The mixed type is more than the cortical type and subcortical type.(5) In this study, stenosis or occlusion of middle cerebral artery and internal carotid artery is most common in patients with supratentorial cerebral watershed infarction, and the frequency of the middle cerebral artery stenosis or occlusion is higher than the internal carotid artery, and the incidence rate of severe stenosis or occlusion of middle cerebral artery is higher than that of internal carotid artery.(6) In the mixed type, the frequency of internal carotid artery and middle cerebral artery stenosis or occlusion is higher than that of the cerebral cortex, and the cortical type is higher than that of the mixed anterior cerebral artery.(7) Qi deficiency, phlegm dampness, Yang quality are the high risk of vascular stenosis. There is no significant difference in the degree of vascular stenosis between different constitutions.(8) The mixed type was higher than the subcortical type in the severe stenosis or occlusion of the vessel.
