Hypertensive disorders complicating pregnancy(HDCP) is the obstetrics unique disease, its disease incidence rate makes 10%[1]. The hypertensions, dropsies and albuminuria are its model clinical manifestation, when is serious will appear twitches, stupor and so on, even caused the pregnant woman, embryo or the perinatal mortality. The HDCP aetiological agent has not been clear, pathogenesis that the endothelial cell activation and damage theory recognize at present. In recent years some relevant research showed that the eclampsia earlier period's hazard factor is in the blood serum the blood vessel tight element II- Type I acceptor own antibody level increases [2]. This study through the observation of fetal left and right ventricular Tei index, atrioventricular cingulum displacement and E/A ratio and to guide clinical intervention in time, at the right time to choose the timing of the termination of pregnancy and childbirth way, in order to reduce the occurrence of adverse consequences.451 sdudies from 20 to 37+6 gestational age were recruited from these who came to our hospital for pregnancy examination. They were divided into 2 groups: the normal group includes 377 fetales without HDCP, the abnormal group includes 74 fetales with HDCP and are confirmed by the clinic. The E8 Ultrasonoscope were used to examine fetales(C4-8-D convex array probe, frequency 2~5MHz). Firstly, examine the fetal structure, estimate gestational age and examine the fetal heart to except heart attack. Left and right ventricular Tei index and E/A ratio obtained by pulsed-wave Doppler mitral. To measure atrioventricular annulus displacement by M-mode echocardiography. Comparison between the twogroups left and right ventricular Tei index, atrioventricular annulus displacement and E/A ratio of change, evaluate the correlation analysis between fetal cardiac function and the parameters.SPSS19.0.0 statistical software was used. The data was expressed as mean ± SD. The t-test and chi-square test were applied in the intergroup comparison. P-value less than 0.05 was considered statistically significant.The relationship between two variables using Pearson correlation analysis.The results showed as follows:1 Maternal age, gestational age and fetal heart rate had no difference between the two groups(P>0.05).2 There was no correlation between left and right ventricular Tei index,atrioventricular cingulum displacement and E/A ratio and their gestational age(LV Tei index: r = 0.020, P > 0.05; RV Tei index:r=0.027,P>0.05; MAD: r=0.087,P>0.05; TAD: r=0.093,P>0.05;Mitral valve E/A ratio: r=0.036,P>0.05; Tricuspid valve E/A ratio:r=-0.017,P>0.05).3 There was no correlation between left and right ventricular Tei index,atrioventricular cingulum displacement and E/A ratio and their heart rate(LV Tei index: r=0.005, P>0.05; RV Tei index: r=-0.025,P>0.05;MAD: r=-0.044,P>0.05; TAD: r=-0.057,P>0.05; Mitral valve E/A ratio: r=0.025,P>0.05; Tricuspid valve E/A ratio: r=0.012,P>0.05).4 Tei index had no difference between fetal LV and RV(P>0.05).5 The Tei index of fetal LV and RV in the abnormal group were higher than those of the normal group(P<0.05).The MAD, TAD and E/A ratio in the abnormal group were lower than those of the normal group(P<0.05).6 Left ventricular Tei index, MAD and E/A ratio of MV combined detection of left heart function that the sensitivity was82.43%, the single left ventricular Tei index monitoring the sensitivity of the left heart function was60.81%. Right ventricular Tei index, TAD and E/A ratio ofTV combined detection of right heart function that the sensitivity was81.08%, the single right ventricular Tei index monitoring the sensitivity of the right heart function was56.76%.7 In summary, the fetus left and right ventricular Tei index,atrioventricular cingulum displacement and E/A ratio of three joint monitoring fetal heart function timely, accurate, comprehensive and reliable enough to reflect the blood circulation of the fetal, can real-time understanding of fetal intrauterine ischemia anoxic condition, for clinical treatment and according to the condition to choose the appropriate delivery time to provide accurate objective basis.
