BackgroundUlcerative colitis is a kind of etiology and pathogenesis colorectal chronic nonspecific inflammatory reaction disease which has not been fully elucidated. The treatment will also be a long-term process of constant exploration. It is necessary to make a long time study, to explore the effective of diagnosis and treatment, to make feasible and convenient method to improve the patients' clinical symptoms and quality of life. It has been known that intestinal mucosal inflammation was caused by abnormal immune system response process. Furthermore,oxygen free radical damage plays an important role in the pathogenesis of ulcerative colitis.Therefore, glucocorticoid and 5-aminosalicylic acid drugs have become the main drugs for the treatment of ulcerative colitis. Most glucocorticoid was applied to the initial treatment for severe ulcerative colitis in the world. But the effect of glucocorticoid applied in mild-to-moderate ulcerative colitis which applied 5-aminosalicylic has no clearly effect is not clearly.ObjectiveTo observe the effect of low-dose and short course glucocorticoid of first treatment on mild and moderate ulcerative colitis.MethodsMild and moderate active ulcerative colitis in our digestive department from February 2008 to November 2014 were our objective.176 were randomly divided into experimental group and the control group, experimental group 88 cases.On the premise of informed consent, and the control group 88 examples, to give the salad oxazine 1.0 g, oral three times a day, while giving conventional enema medications; Experimental group 88 examples, on the basis of treatment in the control group used a prednisolone enema or intravenous drug use.31 days will mail instead of oral prednisolone deweighting until withdrawal. In the short term (10-12 weeks) review of colonoscopy, observing intestinal mucosa healing.To explore the comprehensive evaluation of curative effect by observing the improved clinical symptoms and the colonoscopy performance under mucosa.ResultsIn experimental group,83 cases complete the experiment, the total effective rate was 96.39% (80/83), significant efficiency was 65.06%(54/83), effective rate was 31.33% (26/83), invalid 3.61% (3/83), including white blood cells changes in 1 case, 2 cases of nausea,2 cases with diarrhea,1 case of elevated blood sugar,1 case of a skin rash,1 cases appear have a headache, no moon face and osteoporosis patients; in control group,81 cases complete the experiment, the total effective rate was 81.48% (66/81), the efficiency was 37.04%(30/81), effective rate was 44.44%(36/81), invalid 18.82%(15/81), including white blood cells changes in 3 cases,2 cases of nausea,3 cases with diarrhea,2 cases of a skin rash, no case of elevated blood sugar, have a headache, a full moon face and osteoporosis patients. Effect in these two groups has statistical significance(P<0.01), adverse reactions occurred in these two groups has no statistical significance (P>0.05).ConclusionLow-dose and short course glucocorticoid of first treatment on mild and moderate ulcerative colitis is effective with fewer side effects, the tolerance is good. It is worth clinical study and application.
