Objective(s):The Purpose of this study is to assess the relationship among Leukoaraiosis (LA) and cognitive impairment.In addition. To understand the prevalence correlation of severities and to explore the risk factors of leukoaraiosis. Through this research we want to strengthen the importance of early diagnosis and treatment of Leukoaraiosis in order to improve the quality of life in old people and reduce the cognitive dysfunction and dementia.Methods:we chose 105 patients with leukoaraiosis from the department of nerology at the second affiliated hospital of kunming Medical University from May-u2015 to November,2015. All the cases were divided into two groups.Patients with LA-severe paints (LA-3, LA-4) 45 cases, accounting for 42.9% of the total, and LA-mild patients (LA-1, LA-2) 60 cases, accounting for 57.1%. We had investigate and collected basic demographic characteristics and the disease history of the paints.All the patients cognitive status was assessed with the mini mental state examination (MMSE) and the Montreal Cognitive Assessment (MoCA). All the data was double-entered by epidata and analyzed by SPSS statistical software package. A descriptive analysis, single factor analysis and multiple regression analysis were used.Results:(1) The results of MoCA score obsereved was (23.47±4.883). and the prevalence rate of cognitive impairment (VCI) in LA cases is 69.5%. The results of MMSE score obsereved was (24.44±4.400). and the prevalence rate of cognitive impairment (VCI) in LA cases is 62.9%.The sensitivity and specificity of MoCA scale is higher than the MMSE scale.The sensitivity and specificity of MoCA scale is higher than the MMSE scale. (2)To measure MoCA and MMSE score in LA-severe group and LA-mild group respectively,MoCA and MMSE scores with LA-severe group were significantly lower than the LA-mild group (p<0.05). Spearman correlation coefficient of the degree of cognitive impairment and the LA grades(R=-0.705, P<0.001), statistically significant. (3) The degree of cognitive impairment in patients with LA is related to the parts of white matter damage (p<0.05). The LA-DWML group had significantly lower scores in MoCA/MMSE scores than the LA-PVL group.(4) The analysis of single factor and multiple regression analysis factors both revealed that:LA grades, age, body mass index (BMI), smoking history, alcohol history, cerebrovascular, hypertension, diabetes, high cholesterol and sleep apnea Syndrome associated with white matter damage (P<0.05).Conclusion(s):(1) There is correlation between LA and cognitive impairment, LA can cause varying degrees of cognitive impairment. (2) Spearman correlation coefficient of the degree of cognitive impairment and the LA grades,and the higher LA level, the more severe cognitive impairment. (3) The degree of cognitive impairment in patients with LA is related to the parts of white matter damage (P<0.05). The LA-DWML group had significantly lower scores in MoCA/MMSE scores than the LA-PVL group.
