As a bridge-population, not only increases the risk of HIV infection among MSM, but also increases the risk of HIV spread to the general population.Monitoring and intervention is difficult, the management of HIV positive gay men also are difficult. The surveillance, detection and intervention must do in the gay bathhouse, using the rapid detection methods to improve the detection efficiency, increase the trust of service, shorten the testing time and in a timely manner to supply the service.All problems must be killed in the cradle.Truly improve intervention effect, control the positive rate of lost to follow-up and achieve the goal of reducing new infections.Objectives:By baseline survey, to understanding the MSM who are in the gay bathhouse basic demographic information, habits, behavior characteristics, mental state and early life experiences. It adopted cross-sectional survey method,understand the current HIV prevalence in the population who are in the gay bathhouse in Kunming.And use statistical analysis to find factors that may be relevant risk factors for HIV infection in the bathhouse.At the same time,as a valid basis for specialized theoretical to prevention and control and intervention HIV infection among the MSMsMethods:Cross-sectional surveys, quantitative surveys, qualitative interviews, serology, from January 2015 to January 2016 31, more than 18 years of age, It occurred oral sex or anal sex in Kunming gay bathhouse, and willing to accept HIV counseling and testing. Using non-probability sampling decimated 200 people and survey questionnaires. With the extract information saturation principle did the survey in 20 positive cases and 20 negative qualitative interviews. Using the analysis of logistic regression analysis and other statistical methods to analysis the data..Results:1.The Median age is 38.50 years.Theminimum age is 18 years and the maximum age is 67 years old. Accounting for 97.5%(195/200) is Han. Accounting for 32.5%(65/200) is the college education level. Occupational widely distributed, most of the job are distributed to service-oriented business,accounting for 31.7%(63/200). Monthly income of 4000 yuan per month mainly accounted for 29.5%(59/200). Marital status are unmarried mainly.148 people acknowledged that their sexual orientation are gay-oriented accounted 74.0%(148/200).2.The survey shows, the 75.5%(151/200) are specifically indicated that the activities of gay bathehouse. The primary purpose is to have sex. Activities less than two hours in the bath (including 2 hours) of 34.5%(69/200); activity time in more than two hours, within 6 hours (including 6 hours) of 54.5%(109/200); activity longer than 6 hours of 11.0%(22/200). With occasional sexual relations based.75%(150/200) occurred in oral sex in the last three months of the gay bathhouse, only 8.0%(12/150) said they used a condom during oral sex.74.5%(149/200) have penetrative sex had occurred in the three months ago.92.5%(185/200) admitted that they had homosexual relations outside the bath. In order to have sex in the gay bathhouse,the HIV infection rate was 46.9%(15/32), higher than other purposes.21.5%(43/200) had sex with women through the past six months. Including 76.7%(33/43) have sexual relations with women not using condoms during the recent.17.5%(35/200)people said they have not used a condom in the past six months. It is noteworthy that there are four HIV-positive in this 43 respondents. Only one of them had sexual relations using the condom, and others had never used a condom.Most of them have multiple sexual partners, and various forms of sexual parmers.Only three people have no sextual partner.89.9%(177/197) people admitted that they have male casual partners, In which 36.0%(71/197) people admited to having multiple sexual partners.64.8%(46/71) people both have sexual partners of male and female partners. The remaining 35.3%(25/71 people both have regular male sexual partners and have male casual partners.3. The MSM who is in the Kunming gay baths HIV prevalence is high, higher than Kunming MSM sentinel surveillance of HIV infection . The infection rate was 12.0% (24/200). Kunming residential MSM who in the gay bathhouse HIV infection rate was 13.9%(23/165),and non-residential MSMs have no found HIV infection. The Primary school education group HIV infection is the largest, accounting 75.0%(6/8), and the higher education level, the lower infection rates.Monthly income less than 1,000 yuan and no income had a highest HIV infection rate, reached 50.0%.With the monthly income rise, HIV infection rates have decreased. Gay HIV-positive rate was 14.2%(21/148), higher than bisexual HIV infection 3.8%(2/52). The MSMs who had stabal male sexual partners HIV prevalence was 23.5%(12/51), higher than having no stabal male partners HIV prevalence of 7.4%(11/149). The most recent year have done HIV testing MSM HIV prevalence was 6.7%(10/149), lower than the recent year did not detect the MSM HIV prevalence of 25.5%(13/51).4. According to the suvey, the factors which can influence the MSMs' HIV infection including education level, monthly income, sexual orientation, nature of the partner's risk awareness, gay bath house, the type and number of sexual partners, condom use awareness, intervention content and methods and the time spent in the gay bathhouse. the protective factors:a higher education level, the higher risk awareness of sexual partners, a stabal male sexual partners, condom awareness, obtaining an earlier intervention and whether it can get "peer education" intervention. Risk factors:There may be overnight properties gay baths, multiple sexual partners, more sexual partners, sexual orientation is homosexual and stay too long in the gay bathhouse.Conclusions:1 This study investigated a total of 200 MSMs in Kunming gay bathhouse. Age generally too large, most of them are Han, having a higher educational level.Mainly is the local people of Kunming,.the higher rate of gay identity, marital status to unmarried majority, they have different ways of sexual partners.2 Geting baths information mainly through a friend. To the gay bathhouse mostly for the purpose of sex,but the proportion of sex in the bath is not high. High incidence of oral sex, oral sex and condom use rate is very low. The rate of condom use is higher than out of the gay bathehouse.The MSMs who in the gay bathhouse had diverse sexual partners and condom usage is not high. Who was married had a coexistence of men and women sexual partners.Had a lower rate of condom useage with women.Even after HIV infection with spouse sex never used condoms.3.The MSM who is in the Kunming gay baths HIV prevalence is high, higher than Kunming MSM sentinel surveillance of HIV infection .
