Background and Objective:Pulp disease is caused by the pathological changes of pulp tissue,including pulp inflammation,pulp degeneration and pulp necrosis.At present,the World Health Organization has been focused on the prevention and treatment of dental pulp diseases.Because the onset of clinical symptoms is not obvious,it is often difficult to cause patients to pay attention.It is necessary to understand the patient's condition correctly and treatment with purpose in order to obtain the ideal therapeutic effect.At present,root canal therapy is the main treatment of pulp disease.The key of successful treatment is filling root canal tightly.The purpose of root canal filling is to seal the root canal system,prevent the infection from tissue fluid,microorganisms and bacteria,accelerate the recovery of diseased tissue.If the apical closure is not strict can lead to apical micro leakage and treatment failure.The root canal treatment requires a filling material with good biocompatibility.With the use of the oral microscope and the advances in root canal theory,the success rate of root canal treatment is greatly increased.Dental pulp disease is a major disease affecting human health.At present,many countries in the world have established a health care mechanism for the prevention and treatment of dental pulp disease,we still lack of awareness,attention and timely treatment of the disease,thus affecting the quality of life of patients,increasing their economic burden.From October 2012 to October 2014,we selected patients with dental pulp disease,total 1643 teeth,as the research object,which received root canal treatment in Yantai Affiliated Hospital of Binzhou Medical University(former Muping people's Hospital).First of all,the clinical characteristics of patients with pulp disease were analyzed,and then the treatment effect was analyzed.Hope that through this study to understand the clinical characteristics,distribution,and to take the corresponding measures for different groups of people,reduce the incidence of dental pulp disease.Methods:(1)According to the gender,age,occupation,medical reasons,diagnosis and treatment of patients with pulp disease,analyze the distribution of different gender,different age stages,the reasons for the patient's treatment and the diagnosis results.(2)Root canal therapy for all patients were performed by the same physician,follow-up until 12 months of postoperation.According to the standards of subjective symptoms,clinical examination,X-ray film examination of the patients to evaluate the effect of root canal treatment.(3)To investigate the relationship between gender,age,occupation,distribution of teeth and the clinical effect of treatment.(4)To investigate the effect of conventional root canal therapy on the treatment of different dental pulp diseases.Results: 1.Epidemiological characteristics of dental pulp disease(1)The results of gender analysis showed that there were 930 male and 713 female,male is more than female.(2)Age analysis showed that there is a certain distribution in each age group,but mainly concentrated in 50 to 60 years of age,accounting for the incidence of 37.02%.(3)Occupation distribution shows that the largest number of patients is farmer,accounting for the incidence of 35.07%.(4)The medical reason analysis showed that most of the patients mainly due to spontaneous toothache,accounted for 41.94%.(5)The pathological type distribution results show that the dental pulp disease were ranked in the top three followed by acute pulpitis,Chronic pulpitis and periapical periodontitis,respectively,accounting for the total number of 26.23% and 25.99% and 22.52%.2.Clinical treatment effect of dental pulp disease(1)1643 teeth,936(56.97%)cured,545(33.17%)improved,162(9.86%)failed.The success rate of treatment =(936+545)/1643×100%=90.14%.(2)There were 280 cases of successful treatment in 310 male cases,while 213 cases of successful treatment in 237 female cases.There was no significant difference in clinical results between different genders(?~2=0.030,P=0.861).(3)There were 86 cases of successful treatment in 96 patients of 10 to 20 years old;120 cases of successful treatment in 132 patients of 20 to 30 years;173 cases of successful treatment in 195 patients of 30 to 40 years;267 cases of successful treatment in 294 patients of,40 ~ 50 years old;545 cases of successful treatment in 608 patients of 50 ~ 60 years old;225 cases of successful treatment in 246 patients of 60 to 70 years old;65 cases of successful treatment in 72 patients of more than 70 years old.There was no significant difference in clinical results between different ages(x2 =1.375,P=0.967)..(4)There was no significant difference in the clinical outcome between the patients and the treatment(?~2 = 2.263,P = 0.894).(5)There was no significant difference in clinical results between different tooth distributions.(upper ?~2=5.230,P=0.388).(6)The treatment efficiency of conventional root canal therapy on periapical periodontitis and residual root is lower than the acute and chronic pulpitis and tooth essence allergy.Conclusion: 1.It should be focused on the prevention and treatment of male,middle-aged and agricultural population patients of pulp disease in Muping district.2.The effect of root canal therapy was not related to gender,age,occupation and tooth position,while was related to the type of pulp disease.
