Objective :To discuss the effect of electroacupuncture stimulation at Zusanli and Sanyinjiao on analgesic and spinal p38 mitogen activated protein kinase activation in diabetic neuropathic pain rats.Assess the possible mechanism and provided an effective clinical and theoretical basis for clinicians to treat diabetic neuropathic pain.Methods:120 male SD rats,aged six weeks,weighing 180-200 g,1% streptozotocin intraperitoneal injection to induce diabetic neuropathic pain,induced 96 diabetic neuropathic painful rats.The successful rats were randomly divided into 3 groups(n =32 each): diabetic neuropathic pain model group(group D not treated after success been modeling)and electroacupuncture stimulation group(group EA give electroacupuncture treat after success been modeling),sham electroacupuncture stimulation group(Group A only acupuncture acupoint,but not give electrical stimulation).Recorded the changes of mechanical withdrawal threshold,weight and blood glucose at the day befort acupuncture stimulation and 7 days,14 days,21 days after electroacupuncture stimulation.Randomly selected 8 rats for L4-6 spinal cord to inspect p38 MAPK expression levels in the spinal cord when other result been measured,analysis the correlation of pain threshold and p38 MAPK expression levels.Results:(1)The weight decreased significantly in group EA at T2-T4 than in group D and group A(P<0.05);The weight decreased significantly in group A at T3,T4 than in group D(P<0.05).(2)The blood glucose level decreased significantly in group EA at T2-T4 than in group D and group A(P<0.05);The blood glucose level decreased significantly in group A at T3,T4 than in group D(P<0.05).(3)The pain threshold increased significantly in group EA at T2-T4 than in group D and group A(P<0.05);The pain threshold increased significantly in groupA at T2-T4 than in group D(P<0.05).(4)The p38 MAPK activation level decreased significantly in group EA at T2-T4 than in group D and group A(P<0.05);The p38 MAPK activation level decreased significantly in group A at T2-T4 than in group D(P<0.05)(P<0.05).(5)Pearson correlation coefficient for pain threshold and p38 MAPK is-0.825,it suggest that the two changes for electroacupuncture stimulation in DNP rat were negatively correlated.Conclusion :Electroacupuncture stimulation at Zusanli and Sanyinjiao can improve the pain threshold in DNP rats,the mechanism may be related to the inhibition microglia p38 MAPK expression in spinal cord.
