Objective:To study early change of cerebral cortical morphology voxel-based with magnetic resonance imaging,and analysis its relation with disorder cognitive of mild traumatic brain injury patients.Methods:30 patients of mild traumatic brain injury(mTBI)after five weeks of trauma were evaluated the efficacy and prognosis with MRI.Whole brain image acquisitionsed undergo 3.0TMR scanning with routine T1 WI ? T2 WI ? FLAIR,and T1WI-3D.Process data of T1WI-3D imaging using Freesurfer(Version5.2,Harvard university).In this study,we can effectively eliminate the effect of brain edema after brain injury on the brain volume,so we get the thicker or thinner cortex region as region of interest(ROI).The indexed include the cortical thickness and cortical superficial area.At the same time,30 healthy volunteers were selected as control group.The control group was matched with the mTBI group in age,gender,culture and education background.Before scanning one hour,subjects were tested cognition by Mini-mental State Examination(MMSE).Then we got total score of everyone.ROI cortical thickness and area of two groups were analyzed by two independent sample t-test.MMSE scores of two groups was analyzed by Mann Whitney test.Relation between cortical thickness and total score was analyzed by Pearson test.Results:Only one lesion is detected with per patient,a total of 30,4 cases in left frontal lobe contusion,6 cases in right frontal lobe,3 cases in right frontal-temporal contusion,2 cases in the left frontal-temporal lobe,3 cases of contusion in the left temporal lobe,3 cases in right temporal lobe,3cases of contusion in right temporalparietal,2 cases of contusion in left parietal lobe,2 cases of contusion in right parietal lobe,2 cases of contusion in right frontal-parietal.Software Automatically calculatethe result.Cerebral Cortical Thickness(CCT):the middle temporal gyrus,superior frontal gyrus,top lobe,and olfactory lobe in the left cerebral hemisphere of Trauma group were 2.273 ± 0.279,2.876 ± 0.326,2.151 ± 0.459 and 2.277 ±0.407(mm),the control group were 3.194 ± 0.257,3.282 ± 0.236,3.051 ±0.601 and 2.059 ±0.206(mm),there are significant difference(t =-4.727,-4.398,-5.186,3.494,All P<0.005).central gyrus,superior frontal gyrus,middle frontal back and fusiform in the right brain hemisphere of Trauma group were 2.539 ± 0.323,2.339 ± 0.400,238±0.326 and 2.511 ± 0.257(mm),the control group were 2.974 ±0.242,2.920 ±0.366,2.773±0.252,2.773± 0.252(mm),there was significant difference(t=-4.700,-4.672,-4.157,-3.901,All P <0.005).Cortical superficial area :compared with the control group,there was no significant difference in the area of the cerebral cortex in traumatic group(all p>0.05),except right fusiform area atrophy(t=-2.786,p= 0.008).MMSE scores: Trauma group and control group MMSE scores were 19.1 ± 3.0and 29.3 ± 1.0(former r=0.551,p=0.015;r=0.624,p=0.004).Conclusions:CCT atrophy occurred 5 weeks after mTBI injury,and the degree of atrophy was positively correlated with cognitive impairment.
