Vascular dementia(VD) is a kind of nervous system disease caused by many kinds of reasons.It results in overall decline in cognition, memory, verbal, emotion and disappearance of other aspects of nervous system, which is the main manifestation of chronic diseases. It is the second reason of dementia following Alzheimer disease in the elderly, patients with Alzheimer's accounting for 20% to 30% [1]. China is the most people in the elderly, the prevalence rate of senile dementia was 7.8%—3.7%, and the incidence rate of vascular dementia was 1.1%—3.0%[2]. Insufficient blood supply caused by cerebral vascular diseases to the brain and other factors can lead to cerebral vascular dementia. With the continuous growth of the elderly population, the incidence rate is also on the rise. Currently the treatment of vascular dementia is mainly cholinesterase inhibitors, platelet inhibitors, calcium channel blockers, etc, but the effect is not good. Although current treatment would be able to delay the worsening of the disease, there is not a major breakthrough. It makes a impact on the quality of life of elderly patients and brings a heavy burden to the family and society [3]. In recent years, traditional medicine with its holistic concept and treatment based on syndrome differentiation has a unique advantage in the treatment of vascular dementia.Gastrodia elata in our country has a long history of use, containing a variety of active ingredients. The highest content of the active ingredients in gastrodia elata is gastrodin. Gastrodin is known as sedative hypnotic, anticonvulsant, anti-aging puzzle drug. It can protect brain cells and prevent thrombosis and enhance immunity. It has low toxicity and less side effects. It is not easy to produce drug accumulation and has no drug dependence and addiction, etc. It can play pharmacological effects through multi link and multilocus.This reflects that traditional Chinese medicine is better than western medicine in prevention and treatment of vascular dementia. In this study, the effect of the treatment of vascular dementia was observed by using Tianma Pill, which provides a new method for the prevention and treatment of vascular dementia.This study uses Kunming mice and Wistar rats to establish animal aging and dementia model, while giving the Gastrodia elata pill to intervene. Nimodipine Tablets is used as the positive control drug. To observe Tianma pill : 1, learning and memory ability on the animal model of vascular dementia and the improvement of symptoms of dementia; 2, the impact of platelet aggregation and coagulation time in Wistar rat induced by the collagen, PAF and ADP; 3, the effects on hypoxic injury of nerve cells and hydrogen peroxide injury of vascular endothelial cells; and the study of mechanism of Gastrodia elata pill in the treatment of vascular dementia on the basis of preliminary.The experimental results show:1. Tianma Pill on aging mice induced by D-galactose, under 6g/kg,3g/kg dose, effectively improve the aging mice learning memory ability and aging mice brain tissues, improve the activity of superoxide dismutase(SOD) and reduce the content of malondialdehyde(MDA) in brain tissue.2. Gastrodia elata Pill on cerebral infarction dementia rats, in 4g/kg, 2g/kg dose, enhance the ability of learning and memory of rats with vascular dementia, reduce content of lactic acid(LA) and malondialdehyde(MDA) in the brain tissue of rats with vascular dementia, restrains activity of lactate dehydrogenase and cholinesterase, improve activity of catalase(CAT) in brain tissue, glutathione peroxidase(GSH-Px), superoxide dismutase(SOD) and copper zinc superoxide dismutase(Cu-ZnSOD), promote expression of the brain nerve growth factor(NGF), brain-derived neurotrophic factor(BDNF), glial cell line derived neurotrophic factor(GDNF) and nerve nutrition receptors of TrkA and TrkB(tyrosine kinase receptor). But there was no significant effect on the concentration of glutamic acid(Glu), aspartate(Asp) and amino acid(GABA) in the brain.3. Tianma Pill in the dosage of 1g/kg, 2g/kg and 4g/kg could inhibit the platelet aggregation induced by collagen, PAF and ADP in normal rats.4. Tianma pills in the dosage of 6g / 3g / kg and 1.5g/kg does not affect the normal blood coagulation time in mice, but can prolong the clotting time of blood stasis of model mice.5. Tianma pills have protective effects for rat with cerebral cortex nerve cell hypoxia damage induced by sodium hydrosulfite(Na2S2O4) and with vascular endothelial cells ECV304 damage induced by hydrogen peroxide(H2O2), improve the number of survival and reduce the mortality of the nerve cells and endothelial cells in the harmful environment.In summary, Gastrodia elata Pill on vascular dementia has certain therapeutic and prophylactic effects,The mechanism of action may be related to antioxidant, promoting blood circulation to remove blood stasis and improving brain tissue metabolism.
