The Treatment Of Deep Overbite In Adult Patient With Angel Class ? Dvisoin ? Malocclusion By Using Micro-implant

Objective: To investigate the treatment result of four first premolar extraction,used mini-implant anchorage improve overjet,bite turbo and multi-loop edgewise improve overbite in class ?division 1 malocclusion and explore the facial prefile and occlusal relationship.Materials and methods: Select XX Hostipal in 2014,adult patient admitted with malocclusion example.Female,26,class ?division 1 malocclusion,skeletal class ?,convex profile.Badly oral hygiene,gum red and swollen a little.Permanent teeth,class ?molar relationship and class ?canine relationship both side.Left side first premolar is locked occlusion,right side upper second molar is residual root,endodontic perforation.Upper left side,lower both side have third molar,horizontal ippaction.2mm shifted mandible midline to the left.Crowding degree in upper 15 mm,lower 20 mm,over bite 5mm,overjet 13 mm,spee depth is 7mm.We present a case based on oral and maxillofacial clinical findings,and results of the analysis cephalometric,panoramic radiographs,and the model,through communication with the patient,and selece the final treatment plan for:Removal the residual root and four first premolar,nickel titanium wire and stainless steel wire neat rows of flat dentition.Use mini-implant ancourage improve overjet,bite turbo and multi-loop edgewise improve overbite,adjust molar relationship and canine relationship,close the space,used class ?elastic to adjust the mandibular relations.A week before and after treatment,take cephalometric and panoramic radiograhps,within a clear opening photo and face photo,and take a good research model.By comparion before and after treatment retained the X-ray film,photographs,and models to evaluate the effect of treatment.Results:The treatment duration was 22 months.There was no crowding and dentition was properly aligned and leveled.O verjet and overbite both reduced.Midline is basically right.The molar and canine relationship were both C lass?.Spee curve reduced 10 mm,improved obviously.Comparison between the pretreatment and the posttreatment panoramic ratiographs,full-mouth periodontal and alveolar bone showed no abnormalities,dentition was properly aligned and leveled,root substantially parallel,no obvious root resorption,the left and right sides of the condly symmetry without exception:Comparison of cephalometric X-ray films showed :SN A decreased by 3°,SNB decreased by 1°,ANB decreased by 2°,NA-PA decreased 2°,showed that skeletal class ?had imoroved.U1-NA decreased by 29°,showed that the upper incisor a little lingual side.L1-MP reduced 5 °,L1-NB also reduced 2 °,mandibular incisors were mild retracted and upright.PFH from 62 to 64,AFH not change,FMA from 27 ° to 29 °,It is shown mandibular rotate clockwise,mandible vertical change was good control.Soft tissue also had been improved,Z Angle increase from 65 ° to 67 °,H Angle from 23 °reduce to 16 °,lower face looks better.ULEP decreased 2.8,LLEP reduced 1.4,the lips were retracted effectively.Conclusion: This case were treated by straight wire appliance,maxillary incisors were retracted by maxillary mini-implant anchorage,and through the inclined guide plate to open bite,reduce deep overbite and deep overjet,improve facial profile and occlusal relationship.
