| Objective:With the coming of aging society,the incidence rate of hip osteoarthritis is obvious higher,severe osteoarthritis often leads to develop produce hip malformation and bio-mechanics structural abnormalities,caused serious joint dysfunction.According to the classification standard K-L radiation combined with clinical symptoms of hip osteoarthritis can be divided into earlier,early,development period and later period,the osteoarthritis of advanced stage would lead to destroy hip joint structure,so it often need surgery [1].Total Hip Arthroplasty(THA)is one of the important choice to the osteoarthritis of advanced stage treatments,intra-operative joint normal bio-mechanical structure is especially important on postoperative function recovery.Femoral Offset(FO)is closely related to reconstruct the normal bio-mechanics of the hip.This study focused on the relationship of femoral offset and soft tissue balance and revealed the effect of restoration of FO in the abductor Strengths,postoperative complications,joint function after primary Total Hip Arthroplasty.Methods:We collected 61 cases with osteoarthritis of the hip undergoing unilateral Total Hip Arthroplasty during 2013 to 2015 from affiliated hospital of Da Lian medical university,and follow-up time was 6 to 27 months(mean 14.8 months).The patients were taken astandard X ray piece in preoperative and postoperative,we measured the femoral offset,abductor lever arm,body weight lever arm and the length of lower limbs and recorded them.We weighted the difference value of bilateral femoral offset and divided the cases to two groups(FO restoration and not restoration)based on the difference.We taken evaluation for the abductor Strengths,SF-36 scale and Harris hip score.To evaluate the correlation between postoperative FO and reconstruction of the soft tissue balance,and take the comparative study the various indicators of two groups.Result:The relationship of postoperative femoral offset and abductor lever arm is positive correlation(r = 0.803,P <0.05),and body weight lever arm is weak negative correlation(r =-0.397,P <0.05);The postoperative scores of mental-psychological SF-36 scale in femoral eccentricity restoration and not restoration group have no statistically significant difference(t=0.387,p>0.05),The postoperative scores of physiological SF-36 scale in two groups have significant difference(t=3.408,p<0.05);The postoperative scores of Harris scale in two groups have statistically significant(t =3.173,P<0.05).Compared the FO restoration with the not restoration group,the abductor strength scoring,Harris scoring,SF-36 scale physiological function scoring was obviously improved(P<0.05);The restoration of femoral eccentricity is beneficial to recover the isometric of lower extremities,comparing the two groups,the difference was statistically significant(X2 = 15.788,P < 0.05);The restoration of FO can reduce postoperative complications(P < 0.05).Conclusion:The relationship of postoperative femoral offset and abductor lever arm is closely positive correlation,and body weight lever arm is negative correlation,FO has important influence on the reconstruction of postoperative hip soft tissue balance;The restoration of FO can promote the recovery of joint function after THA.Also,The restoration of FO can reconstruct hip soft tissue balance,improve abductor muscle strengths,recovery ofthe hip joint normal mechanical structure,reduce the postoperative complications and reconstruct the function of hip joint. |
