Objective: To assess left atrial function changes among asymptomatic alcoholics by two-dimensional speckle tracking echocardiography(2D-STE)and real-time three-dimensional echocardiography(RT-3DE).Methods: 30 health male adults without drinking history were used as the group A(control group).92 male asymptomatic alcoholics who is drinking more than 5 years,(90~120g)ethanol or(1000~1500ml)of beer daily,were divided into three groups: group B:(n=30),5~9 years drinking(mean 7±2years).group C:(n=31),10~15 years drinking(mean12±1years).group D:(n=31),more than 15 years drinking(mean18±2years).1.The parameters of the conventional echocardiography were obtained respectively.The conventional echocardiography: left ventricular end diastolic diameter(LVDd),left ventricular end systolic diameter(LVDs),interventricular septum thickness diastolic(IVSTd),left ventricular posterior wall thickness diastolic(PWd),left venteicular ejection fraction(LVEF)and fractional shortening(FS).Left atrial(LA)phasic volumes(max,min and pre-atrial contraction volume)were measured from LA volume waveforms,with which passive emptying(LAPEF)and active emptying(LAAEF)indices were also calculated.2.The parameters of the 2D-STE were obtained respectively.2D-STE: Left atrialstain rate(SRs)during left ventricle(LV)ejection,early diastolic strain rate(SRe)during LV early diastole and peak negative strain rate(SRa)after the P-wave were also obtained by averaging all atrial segments.3.Left atrial maximum volume(LAVmax),left atrial minimum volume(LAVmin),left atrial pre-atrial contraction volume(LAVp),left atrial passive ejection fraction(LAPEF)and left atrial active ejection fraction(LAAEF)were measured with RT-3DE.Results: 1.Conventional parameter: All parameters were no significant difference between group B and group A(all P>0.05).LAVmin,LAVmax,LAVp and LAAEF increased significantly through the comparison of group C with group A and group B(all P<0.05),while LAPEF decreased significantly(all P<0.05).LVDd,LVDs,IVSTd,PWd,LAVmin,LAVmax and LAVp increased significantly through the comparison of group D with group A,group B and group C(all P<0.05),while LVEF,FS,LAPEF and LAAEF decreased significantly(all P<0.05).2.2D-STE parameter: All parameters were no significant difference between group B and group A(all P>0.05).SRa increased significantly through the comparison of group C with group A and group B(all P<0.05),while SRs and SRe decreased significantly(all P<0.05).SRs,SRe and SRa decreased significantly through the comparison of group D with group A,group B and group C(all P<0.05).3.RT-3DE parameter: All parameters were no significant difference between group B and group A(all P>0.05).LAVmin,LAVmax,LAVp and LAAEF increased significantly through the comparison of group C with group A and group B(all P<0.05),while LAPEF decreased significantly(all P<0.05).LAVmin,LAVmax and LAVp increased significantly through the comparison of group D with group A,group B and group C(all P<0.05),while LAPEF and LAAEF decreased significantly(all P<0.05).Conclusion:1.Left atrial function depends on drinking time,2D-STE could be an effective method to assess left atrial function in asymptomatic alcoholics.2.RT-3DE could assess left atrium reservoir,conduit,and booster pomp function effectively.It could reflect left atrial function changes when the drinking time change.It could be an accurate,effective and reliable method to assess left atrial function in asymptomatic alcoholics.
