Objective Though the achievement ratio and survival rates of premature babies,low birth weight infants and other critically ill neonates are increasing,the incidence rate of brain injury rises either.More and more medical research and clinical efforts have being focused on early detection,early intervention and early prevention of neonate's brain injury.Although lots of studies have considered on the relative factors of neonate's brain injury,there was few related reports with large sample.As methods lack the systematic ness,results have too many confounding factors,the reliability of related factors remains controversial.Hereby we put maternal pregnancy?prenatal and postnatal factors into more factors conditional Logistic regression analysis model to systematically study the incidence of neonate's brain injury and its possible related factors,discuss the correlation between these factors and neonate's brain injury,to provide the theoretic foundation for early detection,prevention and treatment for premature infants with brain injury.Method The study was prospectively performed on neonate's birth between June 2012 and June 2013 and received treatment in Dalian children's Hospital.Statistical analyses were performed using SPSS 23.0.All data were described as meanąstandard deviation.Student's test was used to evaluate the difference in numerical variables.A logistic regression analysis was performed on factors which found to be significant in univariate analysis.Results Part 1: A univariate logistic regression analysis on premature infants with brain injury.1)Prenatal factors:multiple pregnancies(?2),umbilical cord anomaly(omphaloproptosis,cord around neck),birth trauma and caesarean birth have statistical significance,P<0.05.2)Neonatal factors: white matter showed increased echogenicity,neonatal asphyxia and infectious diseases(pneumonia)have statistical significance,P<0.05.Part 2: A univariate logistic regression analysis on term infants with brain injury.1)Prenatal factors:premature rupture of membrane(PROM),umbilical cord anomaly(omphaloproptosis,cord around neck)and birth trauma have statistical significance,P<0.05.2)Neonatal factors: birth weight>4.0kg,infectious diseases(pneumonia)and respiratory disturbance have statistical significance,P<0.05.Part 3: A multiple logistic regression analysis on premature infants with brain injury.1)Prenatal factors:multiple pregnancies(?2),umbilical cord anomaly(omphaloproptosis,cord around neck),birth trauma and caesarean birth are risk factors of brain injury,P<0.05.2)Neonatal factors: white matter showed increased echogenicity,neonatal asphyxia and infectious diseases(pneumonia)are risk factors of brain injury,P<0.05.Part 4: A multiple logistic regression analysis on term infants with brain injury.1)Prenatal factors: umbilical cord anomaly(omphaloproptosis,cord around neck)and birth trauma are risk factors of brain injury,P<0.05.2)Neonatal factors: infectious diseases(pneumonia)and respiratory disturbance are risk factors of brain injury,P<0.05.Conclusion The logistic regression analysis model reveals the possible related factors of brain injury: multiple pregnancies(?2),umbilical cord anomaly(omphaloproptosis,cord around neck),birth trauma and caesarean birth in premature infant's prenatal period;White matter showed increased echogenicity,neonatal asphyxia and infectious diseases(pneumonia)in premature infant's neonatal period;Umbilical cord anomaly(omphaloproptosis,cord around neck)and birth trauma in term infant's prenatal period;infectious diseases(pneumonia)and respiratory disturbance in term infant's neonatal period.Therefore neonate's brain injury is a complex outcome of mul-factorial interaction.Understanding these possible related factors have contributed to early detection of neonate's brain injury and step effective interventions,which is of great importance to reduce the incidence and mortality of neonate's brain injury.
