The Research Of Endo-GIA Stapler Union Pancreas' Stump Suture In Distal Pancreatectomy

Object : To study the effect in distal pancreatectomy on the application of Endo-GIA stapler combination of pancreas' stump suture on postoperative pancreatic fistula and other aspects.Method:Collected in January 2009-December 2015 seven years in our hospital for the open distal pancreatectomy.According to the patients with different methods to deal with the pancreatic cut ends can be divided into Endo-GIA stapler union manual plunge into pancreatic stump group and simple manual distal suture group.And compare the postoperative pancreatic fistula between the two groups of occurrence and severity of postoperative pancreatic fistula and other aspects.Result: According to the pre-decided exclusion criteria,were included in 41 patients with distal pancreatectomy of pancreatic tail cutting with ENDO-GIA group(19 cases),22 cases of simple distal suture group.In terms of postoperative pancreatic fistula,ENDO-GIA group was 52.63%(10/19),pure manual suture group was63.64%(14/22),no significant statistical difference(P > 0.05),but according to the grading of the pancreatic fistula,ENDO-GIA group(7 cases level A,2 class level B,1cases level C)and pure manual suture group(2 cases level A,10 class level B,2 cases level C)has statistically difference(P < 0.05).Otherwise,In the postoperative blood transfusion rate,ENDO-GIA group(1 case)and manual suture group(10 cases)also has statistically significant(P < 0.05).Conclusion: In distal pancreatectomy,the use of ENDO-GIA union manual plunge does not reduce the incidence of pancreatic fistula,but can reduce the severity of postoperative pancreatic fistula,and to reduce postoperative blood transfusion rate,thus ENDO-GIA joint distal suture in pancreatic tail resection is safe and effective.
