Obstructive sleep apnea hypopnea syndrom (Obstructive sleep apnea hypopnea syndrome, OSAHS) is a chronic respiratory disease,which is characterized by repeatedly happened complete or incomplete obstruction on airway in sleep, accompanied by intermittent hypoxemia or hypercapnia, sleep structure disorder, etc. Objective:To observe the relationship between the obstructive sleep apnea hypopnea syndrom and the multi-system disease, evaluate the effect, clinical significance and reliability of related test index.Methods:1.To evaluate and analyze the effect of CPAP therapy in the maintaining of periodontal condition in patients with OSAHS; 2.To observe the relationship between obesity and the serum cystatin C level in obstructive sleep apnea hypopnea syndrom (OSAHS) patients.Through the above two aspects combined, explore the relationship between obstructive sleep apnea hypopnea syndrom and systemic diseases, evaluate the effect, clinical significance and reliability of related test index.The specific methods are as follows:(1) 52 cases of OASHS patients,18 clinical diagnosed adult periodontitis patients without OSAHS and 20 healthy individuals(all deprived of fundamental diseases) were selected from our hospital between January 2014 and September 2015 by using the method of random sampling, these patients can be respectively divided into the research group (n= 52), the control group(n=18) and the blank group (n= 16). Research group patients received the CPAP therapy patients for three months, and all the three groups were given generally periodontal maintenance.(2) Totally 90 cases of OSAHS patients and 30 healthy individuals (all deprived of fundamental disease) were selected from PLA General hospital between January 2008 and January 2015 using the method of random sampling. These patients can be divided into 3 subgroups (n= 30) as mild, medium, severe and the control group (n= 30) according to apnea and hypopnea index (AHI). Take a venous blood test and record the sleep parameters with the polysomnography to all groups after admission, then measure and analyze serum cystatin C level and body mass index (BMI) of these groups.Results:(1) The six sites'PD and AL difference of research group before and after treatment in patients were significantly greater than the control group (P< 0.05); The alveolar bone height difference of research group before and after treatment in patients is greater than the control group (P< 0.05).The PD and AL difference before and after treatment and the change of alveolar bone height of blank group has no statistical significance.And(2) AHI, ODI, BMI, and serum cystatin C in OSAHS groups were much higher than the control group (P< 0.05), and increased associated with the severity of OSAHS (P< 0.05), while lowest oxygen saturation (L-SaO2) and mean oxygen saturation (M, SaO2) presented a downward trend. AHI, ODI, serum C of obese OSAHS patients group increased more significantly than those in non-obese group,while L SaO2 and M-SaO2 decreased (P< 0.05).In addition, ODI and BMI were positively correlated with the level of serum cystatin C (P< 0.05). L-SaO2 and M-SaO2 were negatively correlated with the level of serum cystatin C (P< 0.05).Conclusion:(1) CPAP therapy method can improve the ventilation status of patients with OSAHS,as well as improve the patient's periodontal condition to a certain extent(2) Obesity can aggrevate the OSAHS disease and boost level of serum cystatin C in OSAHS patients. BMI were positively correlated with serum cystatin C level of OSAHS patients, suggesting that obesity may increase the renal impairment risk in OSAHS patients.
