Part one the curative effect comparison among three operative mehods to cure OSAHS accompanying micrognathia.Objective:To study the preoperative and postoperative airway variation of patients with OSAHS accompanying micrognathia through three different opearive methods, and compare the effect, so as to provide the basis for the patients to select the operative method. Methods:Divided 50 OSAHS patients into 3 groups according to different surgical methods. Group A receives UPPP, group B receives UPPP+chin advancement, and group C receives UPPP+sagittal splitting and advancement of mandibular ramus+ chin advancement. Evaluate the curative effect in terms of all data collected through preoperative and postoperative sleep monitoring as well as the follow-up results analyzed with statistical methods Results:The UPPP has worst effect on these patients, the UPPP combined with other surgical methods have better effect than the UPPP. The UPPP combined with sagittal splitting and advancement of mandibular ramus and chin advancement have better effect on the airway improvement of patients with OSAHS accompanying micrognathia than the UPPP with chin advance, and also better than UPPP (p<0.05). Conclusions:UPPP combined with sagittal splitting and advancement of mandibular ramus and chin advancement was the preferred treatment for patients with OSAHS accompanying micrognathia, which havebetter effect on the airway improvement.Part twoThe effect of 2 kinds of mandibular surgeries on theupper airway soft tissue of patients with OSAHSObjective:To study the correlation between the movement of bone tissue and that of the upper airway soft tissue through chin advancement, as well as sagittal splitting and advancement of mandibular ramus+chin advancement. Methods:To measure the preoperative and postoperative 3D construction CT of the upper airway soft tissue of two groups and compare the relations between the forward distance of patients’ bone tissue and that of the upper airway soft tissue with different surgical methods. Results: there is positive correlation between the displacement distance of bone tissue and that of the upper airway soft tissue, the bigger the displacement distance of bone tissue is, the bigger the displacement distance of upper airway soft tissue will be. Conclusion:For patients with OSAHS accompanying micrognathia, the displacement distance of bone tissue can be appropriately increased to improve the upper airway soft tissue to achieve the better acurative effect, which can provide a scientific foundation for the selection of clinical surgical methods.
