Clinical Observation Of Four-Valiant Decoction For Well-Being Combined With Immunosuppressor In The Treatment Of Lupus Nephritis At Mild To Moderate Level

Objective The aim of Clinical investigation is to observe effect and saf ety of Four-Valiant Decoction for Well-Being combined with Immunosup pressor in the treatment of Lupus Nephritis at mild to moderate level.Method70 patients with active lupus nephritis were selected and randomly divided into two groups:the united Chinese and western medicine trea tment group(therapy group)and single western medicine treatment group(control group),35 patients for each group.During the treatment 10 patients were siftted out,each group remaining 30 patients.Control group were given criterial prednisone orally taking and cyclophamide impact therapy in injection.Therapy group were given Four-Valiant Decoction for WellBeing with dialecticallyadding other herbs on the base of control group.We observed on the/their symptoms and signs pre-treatment and post-tre atment,Blood-routine,urine-routine,ALB,AST,CREA,ESR,ANA,Anti-ds-DNA,24 hUm Tp,active degree of lupus were measured to evaluate the effect of this two therapeutic schedules.Results Results of the study could be summarized as:(1) Therapy group total effective rate was 86.67%;while control group was 83.33%;There was no remarkable difference between the two groups on the total therapeutic effect(P>0.05).(2)Efficacy of TCM syndrome analysis:the therapy group total effective rate was 86.67%,while control group was 56.67%.therapy group was better than control group on improve efficacy of TCM syndrome(P<0.05).(3)TCM syndrome scores and SLEDAI of the two groups have remarkable reduced than that of pre-treatment(P<0.05). And the TCM syndrome scores of therapy group was less than control group after treatment(P<0.05).(4)After treatment, analysisonroutine blood test,ALB,Scr,Urine-RBC, 24 h Um Tpet. has remarkable improved than pre-treatment(P<0.05).the effect ive of therapy group on reducing Urine-RBC,Urine-protein and increasi ng ALB,RBC,HGB was better than control group(P<0.05).(5) Compared with pre-treatment, the Positive rate of anti-ds-DNA,anti-Sm were decreased,but it was no statistical significance(P>0.05).After tre atment the integral of ANA titer, complement C3/C4, ESR were significantly improved than before treatment(P<0.05).but the there was no dramati c difference between the two groups on these indicator of post-treatment(P>0.05).(6) The time of reducing prednisone was remarkable earlier in therapy group than that of control group(P<0.05).(7) Therapy group has better effect than control group in anti-side-effect of prednisone and cyclophamide such as descent of white blood cell,acne,gastrointestinal disturbances and infections(P<0.05).Conclusion The efficiency of Four-Valiant Decoction for Well-Being combined with Immunosuppressor in the treatment of Lupus Nephritis is better than the single Immunosuppressor therapy,especially in controlling the activity of disease,improving TCM symptoms and reducing the occurrence of adverse reactions. This study provides evidence that this treatment is worth popularizing.
