Clinical Observation Of Application About Ropivacaine Combine Dexamethasone And Pentazocine In Brachial Plexus Block

Purpose To explore the role of ropivacaine combine dexamethasone and pentazocine in brachial plexus block with general anesthesia in arthroscopic surgery.Methods 60 American Society of Anesthesiologists(ASA)physical status ? - ? patients were randomly allocated into three groups with 20 cases each.All patients were performed interscalene brachial plexus block by a nerve stimulator under ultrasound guidance combined with general anesthesia.Patients in group C were given 0.375% ropivacaine20 ml,in group A were given 0.375% ropivacaine 20 ml include pentazocine 30 mg,and in group B were given 0.375% ropivacaine 20 ml include pentazocine 30 mg and dexamethasone 8mg.Patients were proceeded with induction after brachial plexus block worked and none of adverse reactions occurred.Sensory block onset time,motor block onset time and duration of analgesia were recorded.Arterial blood pressure and heart rate before the surgery,10 min after the beginning of the surgery and 10 min after removing the tracheal catheter were recorded.The operation time,extubation time,adverse effects of brachial plexus block(Local anesthetic toxicity,Paresthesia,Dyskinesia,Nausea and vomiting),VAS score at 6h,12 h and 24 h after surgery were also analyzed.Results Sensory block onset time,motor block onset time in group A and group B were shorter than that in group C,and the duration of analgesia were longer than group C.Compared with group A and group C,patients in group B were given less opioid dosage.The VAS score in group A and group B were lower than group C at the time of 6h,12 h,24h after the operation,furthermore,group B was the lowest one at the time of 12 h,24h.Conclusion Ropivacaine plus dexamethasone and pentazocine in brachial plexus block can speed the onset time,prolong the duration of the analgesia,decrease the amount of narcotic drugs and improve patient satisfaction.
